A mechanic at an auto body shop located near Mojave Road and Fremont Street has died after a work accident on Sunday afternoon. According to the Las Vegas Metropolitan Police Department, the man was working on a car when it fell on him, killing him.

One man dead after fall at Shoun Lumber in Butler

JOHNSON COUNTY, TN — According to Johnson County Sheriff Mike Reece a man died just days ago at a lumberyard in Butler. Sheriff Reece told News Channel 11 over the phone the incident happened around 5 p.m. Thursday at Shoun Lumber on George Shoun Lane. He said the man was an employee at the company and was on the job at the time of the accident. “Upon arrival my investigators found that Edward Maines had an accident at the mill,” Reece said. “He had fell on a saw and he was deceased upon arrival.” Sheriff Reece said because this death occurred at a work place it’s now under investigation by both the Occupational Safety and Health Administration and the Tennessee Occupational Safety and Health Administration.

Semi-truck driver killed in single vehicle crash

An investigation is underway after a semi-truck driver is killed in a crash. The crash happened just before 8:00 last night in the city limits of Broken Arrow. Michael Benninghoven, 47, was driving a 2017 Kenworth semi when he failed to negotiate a slight curve to the right. The semi departed the roadway and rolled over. The driver was pinned for two and a half hours before Broken Arrow Fire extricated him.

Worker killed in Raleigh County I-64 construction zone accident

RALEIGH COUNTY, W.Va.  No charges were filed in the immediate aftermath of the death of a construction worker killed in an Interstate 64 work zone in Raleigh County. State Police told WSAZ Television the victim, identified as Nino Garcia, died after being backed over by a service truck one of his fellow workers was driving in the I-64 work zone near Beaver.

Authorities release name of man killed in Dubuque fall

DUBUQUE, Iowa (AP) – Authorities have released the name of a man fatally injured in a fall at a Dubuque construction site. The Jones County medical examiner’s office said Thursday that he’s been identified as 57-year-old Timothy Leibold, who lived in Dubuque. Fire officials say Leibold fell Monday from an upper floor of a hotel being built. The Jones County medical examiner, Dr. Michael Weston, says Leibold died at an Anamosa hospital while in transport to an Iowa City hospital.

Identity released of two workers killed in elevator collapse

NAPLES, Fla. — Naples Police have released the identity of the two people who fell after an elevator collapsed Wednesday.  According to investigators the accident involved two employees of Sunbelt Rentals, Enreique Gonzales, 43, and Wojiech “Walter” Iwansowski, 48. The two men were working on a temporary elevator shaft to the exterior of the condominium when investigators say the shaft failed.  Gonzales and Iwansowki fell approximately 110 feet, killing both men. No other injuries from construction workers or Club Brittany condominium residents were reported.

Truck driver killed in Highway 60 crash

POLK COUNTY, Fla. — A fatal traffic crash involving a single vehicle shut down the eastbound lanes of Highway 60 at Hadden Road, east of Bartow, for several hours. The Polk County Sheriff’s Office responded to the Friday morning crash, which happened at about 6 a.m. A 2016 Hino commercial flatbed truck was hauling metal building supplies when it drifted onto the grass median, according to a witness.The driver appeared to over-correct and rolled over on its roof. The cab of the vehicle collapsed, pinning the driver.

2 dead after truck boom apparently hits Colorado power line

COLORADO SPRINGS, Colo. — Two workers are dead after the boom of their truck apparently touched high-capacity power lines, igniting an explosion, in southern Colorado.The workers (Joseph Anthony Sampedro and Benjamin Hey, ) were taking soil samples near the Ray Nixon Power Plant south of Colorado Springs Thursday when the explosion happened. The Colorado Springs Fire Department says a third worker was not injured.

Local man dies in work-related accident

ALPENA, MI — A 52-year-old man who was an employee of Viatech Corp. in Lachine was killed while at work Tuesday afternoon at about 6:30 p.m. The name of the victim was not released. According to a Michigan Occupational Safety and Health Administration memo the man and another employee were operating a tube-cutting machine and cutting metal tube rollers used for conveyors to size when one of the tubes didn’t cut all of the way through. The man adjusted the RPM to cut another tube, but there was a burr caught on the cutting blade that caused the roller to fly up and strike him in the area of the throat.

New details in fatal Shelby County explosion

Harpersville, GA — New details being revealed tonight about what may have caused a conex unit to explode Wednesday in Harpersville — killing an industrial worker. Investigators are examining two welding machine tanks and one propane tank they removed from the unit after the explosion. “To see if it could be a part of the cause of the explosion, or the cause, says Harpersville Police Chief, Jimmy Macon. “And again it’s still under investigation…and these types of things take time.”  Five workers were on the scene. 36 year old Christopher Anderson died from the blast after opening the door. The company, Legend Directional Service, LLC, will resume laying the gas pipeline Monday. OSHA says the Mississippi-based company has had *no* violations within the past five years. They were hired by a company based out of Texas.

OSHA investigating death of Chicago man at Niles laundry business

Niles, IL — A laundry and cleaning shop in North suburban Niles is being investigated after the death of an employee Tuesday night. A 42-year-old Chicago man died in an industrial accident after becoming trapped between a laundry conveyor belt and a dryer about 10:40 p.m. at EcoDirect Laundry and Cleaners, 6977 N. Austin,  according to the Niles Police. Pascual Gonzalez-Hernandez was found dead at the scene, according to police. Investigators believe he was trying to remove a towel that was hanging from the dryer in front of the conveyor belt when he became stuck about 10 p.m. He was found by another employee when they noticed a washer and the dryer were not working, police said.

OSHA confirms Globe plant death

Selma, AL — The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) confirmed a Globe Metallurgical employee has died after being burned in an incident at the company’s Selma plant last month. Michael D’Aquino, an OSHA spokesperson, confirmed the employee’s death. He said there is an open inspection for the incident, which happened on April 24. Dallas County Coroner Alan Dailey identified the employee as 26-year-old Stephen Jamar Shanks. Dailey said Shanks would have turned 27 at the end of May. He passed away Sunday at UAB Hospital in Birmingham.

Worker dies after a trench collapses at a Lake Forest construction site

Lake Forest, CA — A worker died Wednesday after a trench about 17 feet deep collapsed at a Lake Forest construction site, burying his body underneath dirt, authorities said. Construction crews handling the grading for a tract home project were using an excavator to dig when a side of the trench gave way, said Capt. Larry Kurtz of the Orange County Fire Authority. It’s unclear if the worker, who was not identified, fell inside or was acting as a spotter in the trench, Kurtz said. Two or three dozen people were on-site working for a company called Empire Grading. Fire officials were called to the development, near Viejo Ridge Drive and Glenn Ranch Road, about 3 p.m. They used powerful vacuum trucks and buckets to carefully remove the dirt around the body during the recovery, Kurtz said.

TREE SERVICE WORKER KILLED BY FALLING TREE

Bellefontaine, OH — A Bellefontaine man was killed Tuesday when a tree fell on him while clearing a lot off Pinewood Court. John W. Anderson, 56, was operating a chipper for Brown’s Lawn & Tree Service while other workers were using chainsaws to bring down trees, detectives with the Bellefontaine Police Department said. Emergency crews were dispatched at 9:45 a.m. Tuesday to the lot. Anderson was declared dead at the scene and his body was removed by the Logan County Coroner’s Office for an autopsy.

Dump truck driver killed in one vehicle crash on Swamp Road in Newtown Township

Newtown Township, PA — A dump truck driver was killed Tuesday morning when his rig overturned on Swamp Road and slammed into a telephone pole just south of Twining Bridge Road. Police said Zafer Akar, 33, was driving south on Swamp Road around 11 a.m. when the tri-axle truck, loaded with crushed stone, overturned. The truck impaced a PECO telephone pole knocking out power to the area.

Vehicle explosion kills one man in Lebanon County

East Hanover Township, PA — It was around 9:30 Wednesday morning when disaster struck in the JP Donmeyer INC. parking lot in East Hanover Township, Lebanon County. State police say 69 year-old Michael Floyd of Harrisburg was driving a truck tractor to a back parking lot to hook up a trailer. “Another person that works for the company, he was driving a van down there to essentially pick him up and drive him back up to the building,” said Trooper David Beohm with the Pennsylvania State Police. That person was 65 year-old Stephen Miller of Lebanon. As Floyd got out of the truck to get into the van, the van exploded, killing Miller immediately and sending Floyd to the Hershey Medical Center with non life-threatening injuries from the shrapnel. “So the van that he was in, they call it their maintenance van or something to that effect, it had acetylene and oxygen tanks in it, air compressor, the welding equipment in it. Something you would use where big trucks are at,” said Trooper Beohm.

Truck driver found dead after semi crash on Interstate 15 near St. George

St. George, AZ — A truck driver was found dead Tuesday after authorities responded to calls a semitractor-trailer had fallen off Interstate 15 inside the Virgin River Gorge outside of St. George. Callers reported the truck went down an embankment near mile post 24, six miles north of the Cedar Pocket campground, according to a preliminary report from the Arizona Department of Public Safety. Troopers found the truck in a dry wash, with the driver dead.

Johnson City man killed in work-related accident at Unicoi factory

UNICOI COUNTY, TN (WJHL) – Unicoi County Sheriff Mike Hensley confirmed a Johnson City man died on the job on Monday. Hensley said the man worked at Specialty Tires on Unicoi Drive in Unicoi County.The man, whom Hensley identified as Ben Shew of Johnson City, worked in maintenance for the company. Hensley said the man’s clothing got caught in a machine’s conveyor belt.  Specialty Tires is a tire manufacturer.

Construction Worker Killed in ATV Accident

JEFFERSON DAVIS PARISH, La. –  KLFY reports that a construction worker operating an all-terrain vehicle (ATV) was killed when he traveled into the path of another vehicle near Jennings. According to a preliminary investigation, 27-year-old Drake James Scrantz was operating a Polaris Ranger when he attempted to enter the I-10 South Frontage Road from an adjacent median when he was struck by a westbound Dodge pickup truck. At the time of the crash, Scrantz was attempting to return to a construction yard after conducting road work operations nearby. Authorities say Scrantz, who was not wearing a seatbelt, was ejected from the ATV and died as a result of his injuries. The driver of the other vehicle, who was wearing a seatbelt was not injured.

In regards to fatality listed in this blog on vehicle falling upon mechanic at automotive repair facility in Las Vegas, a similar fatality previously occurred in Ely, Nevada at the Nevada Department of Transportation shop. For hydraulic jacks and hydraulic support equipment, always have a tandem (dual) support back up – preferably a hard metal fixed support that is specifically built for that purpose, like a metal jack stand that has marked support weight capacity on it. Home built or gerry rigged support items (bricks, wood, cans, etc.) will fail. Make sure hydraulic jacks and jack stand have the support capacity to hold vehicle, and make sure this equipment has capacity markings on them to ensure correct support capacities.
