Cambria, WI — Didion announced the passing of Carlos “Charly” Nunez<\/strong> late Friday night. Nunez died from his injuries suffered in the blast<\/a>. Three people died from the initial explosion at the milling facility in May. Since then two others have died from injuries.<\/p>\n

Massilon, OH — Mitchell R. William<\/strong>s loved drag racing, fishing, gaming, classic cars and raced go-carts, according to his obituary. Now the Occupational Safety and Health Administration is investigating his death, which occurred Thursday afternoon while he was with a crew that was mowing at the Ohio Northern University<\/a> Metzger Nature Center in Bucks Township, about 4 miles south of Ragersville. The 18-year-old senior at Perry High School in Stark County had not done any work at the site, according to Tuscarawas County Sheriff\u2019s Deputy Rick Morrison.Morrison said Williams was part of a landscaping crew working for Fimple\u2019s Lawn Care of Massillon. They had started their workday \u2014 Williams\u2019 first with the company \u2014 with a small job in Massillon before completing two others in Tuscarawas County. The university nature center was the fourth job of the day. Mitchell had told another employee he had gotten sick at the second job, Morrison said. \u00a0He was just laying in the grass, unresponsive. We were told by (the employee) that he was still gasping at that point in time. (The employee) attempted to do CPR, as instructed by the 911 operator, but to no avail.\u201d<\/div>\n<\/div>\n<\/div>\n<\/section>\n

Volunteer firefighter dies after injuries from blaze<\/h3>\n
\u00a0NARA VISA, NM \u2014 A Nara Visa volunteer firefighter died<\/a> on Thursday after suffering burns trying to extinguish a fire that was more than seven miles long and three miles wide.\u00a0John Cammack<\/strong>, 74, of Nara Visa, was severely burned after falling from a fire engine during a “burn over” Wednesday night, said Nara Visa Fire Chief Gary Girard. Girard said a second firefighter, Kyle Perez, was also injured during the incident.<\/div>\n

Dump truck strikes, kills construction worker in Happy Valley<\/h3>\n

HAPPY VALLEY, Ore. \u2014\u00a0A dump truck struck and killed a 71-year-old construction flagger<\/a> Friday afternoon in a crash police are calling an accident.\u00a0<\/span>The crash happened near 132nd Avenue and Rose Meadow Drive at about 4 p.m. while NW Natural Gas and the construction crew was working to stop a gas leak. Clackamas County sheriff’s deputies say the dump truck had some kind of mechanical issue that caused it to go into motion. The worker who was killed has not been identified. The driver stayed at the scene. Deputies said neither the dump truck driver nor the flagger were employees of NW \u00a0Natural. Construction sites have been deadly for two other construction workers in recent weeks. Pablo Dominguez-Amezcua<\/strong>, 48, of Portland, was killed May 23 while working a construction<\/a> site on Southeast 122nd Avenue, also in Happy Valley. \u00a0And on June 6, 57-year-old Ronald Davis<\/strong> was in his construction vehicle when was hit by another vehicle <\/a>on Interstate 5 just north of the Donald\/Aurora exit. He died several days later.<\/p>\n<\/div>\n<\/div>\n

Auburndale, Fla — A 33-year-old worker died Wednesday from an apparent electrocution<\/a> at Caribbean Distillers in Auburndale, Fla. The Polk County Sheriff\u2019s Office reported that Aaron Rowe<\/strong> of Kissimmee, Fla., was attempting to fix a conveyor belt and had appeared to turn off power to the line he was working on. He then spliced the wires to make the connection to the motor. Unbeknownst to Rowe, a wire connected to the motor was live, and officials say the lock-out system used to shut off power was affixed to the wrong circuit breaker. Rowe, who suffered burns to his hands, was pronounced dead at Winter Haven hospital approximately one hour after being shocked.<\/p>\n<\/div>\n<\/div>\n

Woman killed by bear in Alaska was newly married<\/h3>\n<\/div>\n

ANCHORAGE, ALASKA —\u00a0<\/span>The employer of an Alaska contract mine worker killed by a black bear<\/a> says the woman was married less than two weeks before her death. The newlywed was 27-year-old Erin Johnson<\/strong> of Anchorage. Her co-worker, 38-year-old colleague Ellen Trainor of Fairbanks, sustained minor injuries in the attack Monday near the Pogo Mine, about 275 miles northeast of Anchorage. Employer Steve Murphy of the environmental services firm ABR Inc. also says he believes Trainor survived the frenzied attack by the “hyper aggressive” bear because she used bear deterrent spray. The women were collecting geological samples as employees of ABR about 5 miles from the Pogo Mine when the mauling occurred. The bear was later killed.<\/div>\n<\/div>\n

One Dead Following Industrial Accident In Allen County<\/h3>\n

DECATUR, IN — \u00a0One person has died and another was taken by helicopter to a local hospital after an industrial accident<\/a> outside Decatur. Adams County Coroner Leslie Cook says emergency crews were called to Manley Meats around 3:30 p.m. Wednesday for a report of two unresponsive people. In a statement, Cook says work was being done on a sewer pit when one or both people became unresponsive. One person was pronounced dead at the scene.<\/p>\n<\/div>\n<\/div>\n

Construction worker dies on the job in Fort Madison<\/h3>\n

FORT MADISON, Iowa – For Madison Police say a worker died at a <\/span>construction site<\/span><\/span> after suffering a fall<\/a> on Wednesday. <\/span>At approximately 3:30 pm on June 21, 2017, Fort Madison Police were dispatched to 4901 Ave O for a fall. Upon arrival, first responders observed a man lying on the ground at the north end of the <\/span>building<\/span><\/span>. <\/span>A witness to the fall said the <\/span>worker<\/span><\/span> fell out of a wooden box that was being lifted by a forklift. The witness approximated that the man fell about 20 to 25 feet. <\/span>The worker, identified as 45-year-old <\/span>Brian Wood<\/strong> of Linn Creek, Missouri, was transported to Fort Madison Community Hospital where he was later pronounced dead.<\/span><\/p>\n

‘Tragic Accident’: PECO Worker Electrocuted While Responding to Power Outage<\/h3>\n

Rosemont, PA —\u00a0<\/strong>A PECO worker died after he was electrocuted while responding to a power outage Wednesday in Rosemont, Pennsylvania. Walter Carter<\/strong> was in an elevated bucked along Berwood Lane near Boxwood Road when he was electrocuted while working on a power line around 5 a.m., Radnor Township police said. A police officer who responded to scene found a worker on the roadway who said his co-worker was inside the raised bucket, police Lt. Andy Block said. Fire crews assisted in lowering the bucket and found the 62-year-old line worker unresponsive inside, police said. He was pronounced dead at the scene by the Delaware County Medical Examiner\u2019s Office.<\/p>\n

GREENFIELD, Wis. \u2014<\/strong>\u00a024-year-old maintenance worker was<\/span> killed Thursday morning when a landscape shed exploded<\/a>, an official said. Firefighters were called to the 7000 block of West Southridge Drive, near the <\/span>Willowick Apartments<\/span>, at about 10 a.m. First responders found the shed fully engulfed in flames and a man on the ground with extensive burns.\u00a0<\/span>Andrew J. Knutson<\/strong> was pronounced dead at Columbia-St. Mary’s Hospital.<\/span><\/p>\n

Chicago Man Dies In Army Corps Industrial Accident In Alaska<\/h3>\n

ANCHORAGE, Alaska (AP) \u2013 The Army Corps of Engineers has identified the man killed on an Alaska construction project as a Chicago employee on temporary assignment. Melvin McLaurin<\/strong> died Monday afternoon while operating heavy equipment <\/a>on the Chena River flood control project outside North Pole, Alaska. McLaurin worked as an engineering equipment operator lead with the Corps\u2019 Chicago District. The agency says McLaurin was operating a roller compactor on a stability berm and preparing the site to install a relief well. The compactor rolled just before 4 p.m. Emergency responders declared McLaurin dead about a half hour later.<\/p>\n

Semi truck driver killed in fiery crash with freight train in Benton Co.<\/h3>\n

BENTON COUNTY, Wash. – A truck driver has died after a freight train slammed into his semi-trailer<\/a> rig as it was crossing tracks in Eastern Washington. Emergency personnel responded to the scene, near Plymouth in Benton County, at about 3:30 p.m. Wednesday after receiving reports of a train derailment. \u00a0Officials told KEPR-TV News that responders found what was left of a semi attached to the first train engine in flames. Medics found the driver, 66-year-old Ronald Slagg<\/strong>, to be critically injured. He was airlifted to the hospital but died from his injuries Wednesday evening.<\/p>\n

LINTHICUM, MD \u2014 Anne Arundel County Fire Department officials say a worker die<\/a>d Tuesday afternoon in an accident at Goodwill in Linthicum. The victim fell from a forklift about 2 p.m. at Goodwill Industries, 800 Central Ave. The department said in its blog that witnesses said the employee was operating the forklift when it flipped over and he was pinned under the machine. Coworkers freed the operator, a man believed to be in his early 50s, from underneath the forklift before firefighters arrived. The patient was declared dead by paramedics.<\/div>\n<\/header>\n

SALTILLO, Miss.\u00a0\u2014 A bank teller (Judy Guin<\/strong>)in Mississippi was killed during an attempted robbery<\/a>, and authorities say the suspect took her own life after a police chase. Itawamba County Sheriff Chris Dickinson tells the\u00a0Northeast Mississippi Daily Journal\u00a0a teller was killed Wednesday at the Farmers and Merchants Bank in Mantachie, a town of about 1,100 residents.<\/p>\n

Jasper man killed in rail haulage accident at Oak Grove Mine<\/h3>\n

BIRMINGHAM, Ala. (WIAT) \u2014 A Jasper man lost his life Monday in an industrial accident<\/a>. According to the Seneca Coal company, Marius Y. Shepherd,<\/strong> 32, was fatally injured in a rail haulage accident at their Oak Grove Mine around 6:30 p.m. on June 19. Information from the Mine Safety and Health Administration indicates the accident happened underground. The federal Mine Safety and Health Administration and the Alabama Department of Labor are investigating, and a security guard at the mine said operations are on hold. Shepherd is the 9th coal mine fatality in the U.S. in 2017.<\/p>\n

Second pedestrian fatality in Hebron near Amazon facility, sidewalk project underway<\/h3>\n<\/section>\n

Hebron, OH — An Amazon worker was hit and killed outside the company’s facility <\/a>in Hebron early Tuesday, according to the Boone County Sheriff’s Office. This is the second pedestrian fatality on Litton Lane in recent years. Deputies with the sheriff’s office were investigating in front of Amazon’s main trucking entrance in the 2200 block of Litton Lane off North Bend Road where Paula J. Newsom<\/strong>, 52, of Cincinnati, was struck by a concrete\u00a0mixer, they said. Newsome had just left work and was crossing Litton Lane to an overflow parking lot when she was struck by the mixer owned by Ernst Concrete Services and driven by Trannis Morgan, 66, of Lawrenceburg, Indiana.<\/div>\n

Mayview firefighter killed, another seriously injured in crash at fire scene<\/h3>\n

\u00a0Mayview, Mo — A firefighter with the Mayview, Mo., Fire Protection District was killed<\/a> and a second firefighter was seriously injured in a crash while at the scene of a fire Monday.\u00a0Jeffery M. Sanders<\/strong>, 55, died at the scene and Jacob J. Hayward, 33, was flown to a Kansas City hospital, according to the Missouri Highway Patrol. The three-vehicle crash happened about 6 p.m. south of Mayview along Route E in Lafayette County while Sanders and Hayward were outside of their vehicle. Mayview is 45 miles east of Kansas City. During the crash, the two firefighters became trapped under one of the vehicles as it went down an embankment. Sanders was pronounced dead at the scene, according to the highway patrol.<\/p>\n<\/div>\n<\/div>\n

Worker electrocuted, Littleton residents without power<\/h3>\n<\/section>\n

LITTLETON, CO – Some Littleton residents are without power after a worker trimming trees was electrocuted<\/a>. The man did not survive. \u00a0According to Littleton Fire, the man was with a crew working in a large tree on a sloped lawn. There were power lines nearby. He was about 30 feet up when the accident happened.<\/p>\n

Worker Electrocuted at Commercial Building in Livingston, Report States<\/h3>\n

LIVINGSTON, NJ \u2014\u00a0A worker was electrocuted in a commercial building<\/a> at 53 West Mt. Pleasant Avenue, according to a report by the Breaking News Network (BNN) on Monday afternoon.\u00a0This incident has not yet been confirmed by police nor has the individual been identified. BNN, which monitors emergency radio activity, reported the paramedics pronounced the death of the individual and that the Occupational Safety and Health Administration (OSHA) was notified.
