Authorities said Monday 55-year-old <strong>Richard Fultz<\/strong> was pinned between a pickup truck and a tool chest that was up against a concrete wall when the owner of Backstreet Auto accidentally stepped on the truck&#8217;s accelerator.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<h3 class=\"headline\">Ocala man dies in train accident at Kirby Family Farm<\/h3>\n<p class=\"article-summary\">Ocala, FL<strong>\u00a0&#8212; Anthony Osburn-Day<\/strong> tried to get on the moving train \u2014 which is used for events at the farm \u2014 but fell between the two cars. A 24-year-old Ocala man died while trying to get onto a moving train at Kirby Family Farm in Williston. The man who died, Anthony Osburn-Day, was volunteering at the nonprofit farm, according to Florida Highway Patrol officials. He fell between the train cars on Tuesday evening, was struck by one of the cars and died. Troopers said the farm was preparing for Halloween-related events and they were having trouble with the engine on the restored locomotive. They were testing it when the accident occurred.<\/p>\n<h3 class=\"headline\">Habitat for Humanity volunteer dies at build<\/h3>\n<p>Burlington, NC &#8212; A volunteer working on a Habitat for Humanity project <a href=\"http:\/\/www.thetimesnews.com\/news\/20171006\/habitat-for-humanity-volunteer-dies-at-build\">died Thursday afternoon during a build <\/a>at 1719 Belmont St. in Burlington. The cause of the death of <strong>Ron Peterson,<\/strong> however, remains unknown at this point. Robin Wintringham, executive director of Habitat for Humanity in Alamance County, said members of the nonprofit organization are mourning his death as Peterson was a long-time volunteer and friend. Peterson, 72, was from Gibsonville. Dolores Quesenberry, a representative of the N.C. Department of Labor\u2019s Occupational Safety and Health Division, said preliminary information showed Habitat officials reported the death Friday. However, OSH does not have jurisdiction over volunteer-death situations, only in employer-employee accidents. Peterson apparently was working on scaffolding and fell 6 to 8 feet, Quesenberry said. He died after being taken from the scene.<\/p>\n<h3>Roofer dies from fall in Hamilton<\/h3>\n<p>HAMILTON, Ohio (WKRC) &#8211; A roof inspector was <a href=\"http:\/\/local12.com\/news\/local\/roofer-dies-from-fall-in-hamilton\">killed when he fell through the roof<\/a> in Hamilton Friday. The Butler County Coroner identified him on Monday as <strong>Glenn Griffin,<\/strong> 63. Hamilton Police say Griffin was inspecting the roof of a home with another person when he fell through a weak spot in the roof and fell to a concrete floor.<\/p>\n<h3 id=\"newsStoryTitle\">Man pinned beneath 18-wheeler at Texas UPS facility dies<\/h3>\n<p><span id=\"dateline\">SAN ANTONIO, Texas (CNN)<\/span>\u00a0A man died <a href=\"http:\/\/www.kwtx.com\/content\/news\/Man-pinned-beneath-18-wheeler-at-Texas-UPS-facility-dies-451279663.html\">after he was pinned beneath the tire of an 18-wheeler<\/a> early Tuesday at a UPS facility in San Antonio.The accident happened at around 5:30 a.m. Tuesday as a driver loaded packages into the back of the 18-wheeler. The rig began to roll and the driver ran toward the cab in an attempt to stop it, but ended up pinned beneath one of the tires. He died at the scene.<\/p>\n<h3 class=\"entry-header\">Worker killed in Napa crash was Missouri volunteer firefighter<\/h3>\n<p>NAPA COUNTY, CA\u2014 A worker who\u00a0<a href=\"http:\/\/kron4.com\/2017\/10\/16\/worker-killed-while-helping-crews-battling-fire-threatening-napa-winery\/\">died in a crash while helping North Bay fire crews<\/a>\u00a0in Napa Monday morning has been identified as a volunteer firefighter from Missouri. 38-year-old <strong>Garrett Paiz<\/strong> died when he crashed while driving a water tender just before 7 a.m. on Oakville Grade, according to Napa County Coroner\u2019s Office. He was assisting firefighters as flames threatened the Robert Mondavi Winery near the Oakville Grade and Highway 29.<\/p>\n<h3>Northern Minnesota man killed when house he was moving falls on him<\/h3>\n<p>SHOTLEY TOWNSHIP, Minn. (KMSP)\u00a0&#8211; A northern Minnesota man was<a href=\"http:\/\/www.fox9.com\/news\/northern-minnesota-man-killed-when-house-he-was-moving-falls-on-him\"> killed Sunday when a house he was helping to move onto a foundation fell<\/a>, trapping him underneath. The accident occurred shortly before 11 a.m. in Shotley Township, about 60 miles north of Bemidji on the south shore of Upper Red Lake. A small home was being placed on a foundation when the house fell from the jacks and blocks being used to lift and support it, according to the Beltrami County sheriff\u2019s office. One of the workers was trapped underneath. Emergency crews were able to extricate the worker from under the building and initiated lifesaving efforts, but were unsuccessful. The worker, identified as 25-year-old <strong>Joshua O\u2019Brien<\/strong> of Blackduck, was pronounced dead at the scene.<\/p>\n<h3 class=\"article-headline--title\">Gas station employee shot, killed in York<\/h3>\n<p class=\"headline\">YORK, Pa. \u2014Police in York say an employee of an Exxon Mart was <a href=\"http:\/\/www.wgal.com\/article\/police-gas-station-employee-shot-killed-in-york\/13033823\">shot and killed early Tuesday morning during an attempted robbery<\/a>.\u00a0It happened along the 1000 block of West Market Street. Brand<\/strong> died after being thrown forward from the tractor he was operating on steep terrain and then run over, reports the Kootenai County Sheriff\u2019s Office. Sheriff\u2019s Office and Worley Fire personnel responded to 23100 S. U.S. 95 after receiving a report at 10:10 a.m. of an industrial accident. The incident is still under investigation, per the Sheriff\u2019s Office.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<h3 class=\"headline\">Construction worker killed at solar farm project near Covington<\/h3>\n<p>Covington, OK &#8212; A <a href=\"http:\/\/www.enidnews.com\/news\/construction-worker-killed-at-solar-farm-project-near-covington\/article_29b0c206-b2ad-11e7-8549-6fe4286bc78c.html\">construction contractor was killed<\/a> around 10 a.m. Monday while working at the solar farm project near Covington. Garfield County Sheriff&#8217;s Office\u00a0Sgt. Logan Niles identified the victim as <strong>Davis Knox<\/strong> of Blanchard born in October 1961. \u201cUpon arrival, they located a male subject pinned underneath the trenching machine,\u201d Niles said. \u201cHe was pronounced dead at the scene.\u201d\u00a0Niles said the investigation is ongoing but that Knox may have tripped or his clothing got caught on the machine and pulled him under. There were no witnesses to the accident.<\/p>\n<div class=\"story-head-wrap padding-horz-half-sm\">\n<h3 class=\"story-heading\">Tree worker dies in accident east of Groveland<\/h3>\n<\/div>\n<div class=\"main-story relative clearfix\">\n<div class=\"story-body padding-horz-half-sm\">\n<div class=\"article-body relative\">\n<p dir=\"ltr\">Groveland, CA &#8212; A tree worker <a href=\"http:\/\/www.uniondemocrat.com\/localnews\/5676669-151\/tree-worker-dies-in-accident-east-of-groveland\">died Monday morning in an accident <\/a>outside a home east of Groveland. <strong>Jason Alan Stuart<\/strong>, 35, was a resident of Bay Point, an East Bay community in Contra Costa County.\u00a0 The company is local, from Groveland, and it\u2019s called Down to Earth Construction. Workers were doing tree work and according to the company owner, Stuart was about 80 to 100 feet up a large Ponderosa tree. \u201cHe had on his climbing gear and for unknown reason the other workers heard him say something and looked up and saw Stuart already half the way down the tree falling.\u201d<\/p>\n<\/div>\n<\/div>\n<\/div>\n<section id=\"module-position-QWIpLkddFUQ\" class=\"storytopbar-bucket story-headline-module story-story-headline-module\">\n<h3 class=\"asset-headline speakable-headline\">Point Pleasant man dies in fall at Franklin work site<\/h3>\n<\/section>\n<section id=\"module-position-QWIpLkcKamw\" class=\"storytopbar-bucket priority-asset-module story-priority-asset-module\"><\/section>\n<section id=\"module-position-QWIpLkcch5g\" class=\"storytopbar-bucket story-byline-module story-story-byline-module\"><\/section>\n<p class=\"speakable-p-1 p-text\">FRANKLIN, NJ &#8211; Township police were kept busy Tuesday morning\u00a0responding to two emergencies, including <a href=\"http:\/\/www.mycentraljersey.com\/story\/news\/crime\/jersey-mayhem\/2017\/10\/17\/point-pleasant-man-dies-fall-franklin-work-site\/773463001\/\">one in which\u00a0a 39-year-old man died<\/a>. According to police, around 7:04 a.m. the Franklin Township Police Department received a 911 call about an industrial accident at 125 Claremont Road, where a new elementary school is being built. When police arrived, they found <strong>Peter Martorana,<\/strong>\u00a039, of Point Pleasant at the base of a building, unconscious and unresponsive. Police initiated lifesaving efforts until EMS personnel arrived and took over Martorana&#8217;s care. Police said\u00a0Martorana was working on the roof of the building and\u00a0and fell about 30 feet to the ground. He was transported to a\u00a0hospital, where he was pronounced dead, police said.<\/p>\n<div id=\"WNStoryHeader\">\n<h3 class=\"\">Jackson, MO truck driver killed after abrupt stop<\/h3>\n<\/div>\n<div id=\"WNStoryBody\" class=\"withRelated left\"><span id=\"WNStoryDateline\">FREDERICKTOWN, MO (KFVS) &#8211;\u00a0<\/span>A man from Jackson, Missouri was<a href=\"http:\/\/www.kfvs12.com\/story\/36607424\/jackson-truck-driver-killed-after-abrupt-stop\"> killed in a crash<\/a> in Madison County, Mo. on\u00a0Monday\u00a0morning, Oct. 16. An online crash report from the Missouri Highway Patrol identifies the victim as <strong>Samuel Kranawetter<\/strong>. The report states that Kranawetter, 23, made an abrupt stop on Highway 72 in front of Fredericktown High School. The sudden stop caused aluminum beams he was hauling to shift forward through the front of the trailer and into the rear of the tractor. Kranawetter was pinned by the beams. He died at the scene.<\/div>\n<h3>Dump truck driver killed when truck overturns on quarry access road<\/h3>\n<p>Deposit, N.Y &#8212; The driver of a dump truck <a href=\"https:\/\/www.aggman.com\/dump-truck-driver-killed-truck-overturns-quarry-access-road\/\">died when his truck crashed<\/a> in Deposit, N.Y. at approximately 9:30 a.m. on Friday, October 13, 2017, according to a\u00a0media release\u00a0from the New York State Police, who responded to a motor vehicle accident report with serious injury on a\u00a0<b>quarry<\/b>\u00a0access road off state Route 8 in Delaware County. When troopers arrived at the scene, they found an overturned 1990 Mack dump truck in the middle of the road. After an investigation of the scene, troopers concluded that the truck driver, 66-year-old\u00a0<strong>William B. Barse<\/strong>, was hauling stone from the quarry when he drove off the road several times, striking trees, and finally overturning in the middle of the road. The driver had been ejected from the truck and was pronounced \u201cdeceased at the scene.\u201d\u00a0 The truck is registered to SLLC Inc., a transportation and shipping company.<\/p>\n<h3>SD Fire Chief Dies Battling Structure Fire<\/h3>\n<p>Presho, SD &#8212; Several fire departments in South Dakota have offered condolences following the <a href=\"http:\/\/www.firehouse.com\/news\/12374494\/lodd-presho-sd-fire-chief-donny-manger-killed-in-fire-firefighter-news\">line-of-duty death of a fire chief.<\/a> The U.S. Fire Administration on Monday announced the death of Presho Fire Department <strong>Chief Donald F. &#8220;Donny&#8221; Manger,<\/strong> 54, who fell ill Saturday at the scene of a fire at an unoccupied structure and later died at a hospital.<\/p>\n<h3 class=\"headline\">Subcontractor who died in fall at North Charleston Mercedes-Benz Vans plant identified<\/h3>\n<p>North Charleston, SC &#8212; Authorities have identified an employee for a subcontractor who<a href=\"http:\/\/www.postandcourier.com\/news\/subcontractor-who-died-in-fall-at-north-charleston-mercedes-benz\/article_2f6e1b86-b112-11e7-8e24-87d8032bcfea.html\"> died in an incident<\/a> at the Mercedes-Benz Vans plant construction site in North Charleston. <strong>Michael Thomas, Jr.<\/strong>, a 38-year-old Alcolu resident, died as a result of injuries sustained in a fall, the Charleston County Coroner&#8217;s Office said on Saturday.<\/p>\n<div class=\"row\">\n<div id=\"story-top\" class=\"col-12 story-top-row\">\n<section class=\"storytopbar-bucket story-headline-module\">\n<h3>Hampton convenience store employee killed; owner shot and in critical condition<\/h3>\n<p>Hampton, VA &#8212; The owner and an employee of a Hampton convenience store were shot outside the store at closing time Monday night \u2014 with one killed and the other now fighting for his life. The double shooting took place just before 11:40 p.m. at the Citgo gas station\u2019s Happy Shopper convenience store at 1255 N. King St., about a half-mile from Mercury Boulevard. The employee, <strong>Darrell Wayne Higginbotham,<\/strong> 58, was shot multiple times in the parking lot and found dead there,\u00a0<a id=\"ORGOVV00021\" title=\"Hampton Police Division\" href=\"http:\/\/www.dailypress.com\/topic\/crime-law-justice\/law-enforcement\/hampton-police-division-ORGOVV00021-topic.html\">Hampton police<\/a>\u00a0said. Deputies received a call about the worker at 6:45 a.m. They say the employee died at the scene.<\/p>\n<h3>Concrete slab kills construction worker from Akron<\/h3>\n<p>CLEVELAND, OH &#8212; The,Occupational Safety and Health Administration is investigating the <a href=\"https:\/\/www.ohio.com\/akron\/news\/local\/concrete-slab-falls-killing-akron-construction-worker\">death of a construction worker<\/a> who was killed when a concrete panel fell on him from an aerial lift in Cleveland. <strong>Joseph Matulis<\/strong>, 56, of Akron, was pronounced dead at the construction site at Cleveland State on Wednesday. An OSHA spokesman said Thursday that Matulis was working for a Cleveland company called Coleman Spohn that\u2019s involved in work on a building on the campus. The project\u2019s general contractor, Rhode Island-based Gilbane Building Co., said Matulis was a pipe fitter.<\/p>\n<h3 id=\"head\">New Orleans chief: Officer killed during struggle<\/h3>\n<p class=\"p402_premiumInside\">NEW ORLEANS, LA &#8212; A New Orleans <a href=\"http:\/\/www.dailyherald.com\/article\/20171013\/news\/310139938\">police officer was fatally shot<\/a> Friday during a struggle after he and his patrol team left their cars to investigate something suspicious shortly after midnight. Other officers returned fire, wounding a suspect who eventually surrendered to a SWAT team, police said. Police Superintendent Michael Harrison had said early Friday that officers were fired on as they left their patrol cars. He said that was based on general statements from the officers, who have not been formally questioned about the incident in which <strong>Officer Marcus McNeil<\/strong> was killed and Darren Bridges, 30, was wounded, he said.<\/p>\n<h3 class=\"headline\">Worker killed by truck at Pennsylvania Amazon warehouse<\/h3>\n<p>BOILING SPRINGS, Pa.\u00a0&#8211; The federal Occupational Safety and Health Administration is investigating the <a href=\"http:\/\/www.wpxi.com\/news\/pennsylvania\/worker-killed-by-truck-at-pennsylvania-amazon-warehouse\/612569242\">death of a worker<\/a> at an <a href=\"http:\/\/Amazon.com\" class=\"autohyperlink\">Amazon.com<\/a> warehouse in Pennsylvania. The attempted breakout from the prison in Elizabeth City began around 3 p.m. when inmates set a fire in the prison\u2019s sewing plant, where about 30 inmates produce embroidered logo items, safety vests and other items for government agencies and nonprofits, the North Carolina Department of Public Safety said.\u00a0DPS identified the employees who died as Correctional Officer<strong> Justin Smith<\/strong>, 35, who provided security in the sewing plant, and Correction Enterprises manager <strong>Veronica Darden<\/strong>, 50, who supervised inmates working in the plant.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<h3>Truck driver killed in collision with SUV in Wadsworth: sheriff&#8217;s office<\/h3>\n<p>Wadsworth, IL &#8212; A Kenosha truck driver <a href=\"http:\/\/www.chicagotribune.com\/suburbs\/lake-county-news-sun\/news\/ct-lns-wadsworth-fatal-crash-st-1013-20171012-story.html\">died after a collision<\/a> Thursday morning on Russell Road in Wadsworth that shut down the roadway to traffic for over five hours, according to the Lake County Sheriff&#8217;s Office. The 65-year-old trucker, whose identity was not released by sheriff&#8217;s officials, was driving a Volvo tractor trailer westbound on Russell Road near Park Lane around 5:10 a.m. when it was struck by a Jeep Compass driven by a woman from west suburban Geneva, sheriff&#8217;s office Sgt. Christopher Covelli said in a statement. The statement added that the Jeep crossed the roadway&#8217;s double yellow center line for an unknown reason and struck the tractor trailer, causing it to go off the road and into a ditch.<\/p>\n<h3 class=\"article-headline--title\">U.S. Post Office truck driver killed in fatal Greensboro crash<\/h3>\n<p>GREENSBORO, N.C. \u2014\u00a0A U.S. Post Office driver was <a href=\"http:\/\/www.wxii12.com\/article\/us-post-office-driver-killed-in-fatal-greensboro-crash\/12840664\">killed in fatal crash<\/a> in Greenboro Tuesday night. Greensboro police said <strong>Darryl Pleasant<\/strong>, 35, of Greensboro, was driving a U.S. Postal Service truck westbound on Burlington Road, at about 5:30 p.m. when the crash happened. By 2 p.m., the man\u2019s body was recovered by Clark County Fire Department emergency personnel.
