Rumpke worker dies after being pinned in Middletown crash<\/h3>\n

MIDDLETOWN, OH \u2014\u00a0<\/span><\/b>The Rumpke employee who was pinned between the back of a trash truck and a pickup truck has died, according to the Ohio Highway Patrol.\u00a0A preliminary investigation shows that David J. Evans<\/strong>, 52, of Cincinnati, had stopped the Rumpke truck on Central Avenue. While he was at the rear of the truck collecting trash, a tan 2002 Chevrolet Avalanche, operated by Brian A. Jankovich, age 41, of Middletown, struck Evans and the rear of the Rumpke truck, according to the Ohio Highway Patrol. Evans was transported via Air Care to Miami Valley Hospital where he was pronounced dead.<\/p>\n

PHILADELPHIA, PA (WPVI) –Investigators say a 56-year-old man was pronounced dead after an industrial accident<\/a> in South Philadelphia Saturday afternoon. Police say the man was severely injured while doing electrical work in a customer’s backyard along South Marshall Street.<\/p>\n<\/div>\n

Minnesota truck driver killed in crash in Iowa<\/h3>\n

Decatur County, IA –A Minnesota semi-truck driver was killed in a crash<\/a> on Interstate 35 in Iowa on Thursday.\u00a0 The crash report from the Iowa State Patrol says 62-year-old Paul Charles Griebel<\/strong>, 62 of Savage died when he crashed into a truck pulling a directional arrow sign on a trailer that was advising drivers to move to the left lane.\u00a0 Griebel failed to move over struck the trailer before entering the ditch and rolling. He was not wearing a seat belt.<\/p>\n

Construction Worker Killed in Trench Collapse in Daly City<\/h3>\n

Daly City, CA — A construction worker was killed in a trench collapse<\/a> at a construction site in Daly City, authorities said Friday. The incident was reported 11:20 a.m. at a new housing development on 118 Linda Vista Drive, near Robertson Intermediate School and west of Highway 101. Authorities said the worker was installing a 24-inch concrete storm drain about 12 feet deep when the trench collapsed. Two search and rescue teams were on scene late Friday attempting to recover the worker’s body. “They’re trying to remove some of the dirt and debris from the area to stray and stabilize to allow the rescue workers to go in to extricate the victim,” said Inspector Clyde Preston from the North County Fire Department. The worker,\u00a0Abel Sauceda Quinonez<\/strong>, 34, was a father of four<\/a>.<\/p>\n

Police say the victim was trying to guide the forklift’s driver when the accident happened. The driver lost sight of the man who got between the front and back tires of the forklift, striking him.<\/div>\n

The man fell from the roof of 211 E. Corrections Officer Joseph Gomm<\/strong> was killed last week after an inmate allegedly attacked him at the state’s prison in Stillwater. State Corrections Commissioner Tom Roy is to lay out “next steps” on security on Friday.\u00a0 Authorities say 42-year-old inmate Edward Muhammad Johnson is suspected of attacking Gomm with a weapon. Johnson has been serving time on a nearly 29-year murder sentence from 2002. The union representing Minnesota prison employees has said prisons are understaffed, and state records show assaults on officers have been increasing. King off the roadway. Trooper Tyler Edenhofer<\/strong>, 24, was shot and killed during the struggle. He was in his last week of field training.<\/p>\n

Santee, CA — A 40-year-old pool maintenance worker was found dead Thursday in an apparent accidental drowning<\/a> in a backyard pool in\u00a0Santee. Sheriff\u2019s deputies and Santee firefighters were summoned to the residence off Chaparral Drive around 2 p.m. when it was reported that a man was floating face down in the shallow end of the pool, San Diego County sheriff\u2019s Sgt. Phillip Beaumont said.\u00a0When firefighters pulled the victim out of the water, his body was already beginning to stiffen, though it was unclear when or how he drowned, sheriff\u2019s officials said. LAPD Says Trader Joe’s Employee Was Killed by Officer’s Bullet in Standoff<\/h3>\n

Los Angeles, CA —\u00a0Melyda Corado,<\/strong> a\u00a0Trader Joe’s assistant manager who died during a\u00a0three-hour standoff<\/a> between a gunman and police on Saturday, was killed by a\u00a0Los Angeles Police Department officer’s bullet, the LAPD said Tuesday. “As chief of police, I am sorry for the loss, this tragic loss,” Los Angeles Police Chief Michel Moore said in a news conference.\u00a0“I want to express my deepest condolences and sympathy to her family and to everyone who knew her.”\u00a0 A\u00a0standoff\u00a0ensued when a gunman took\u00a0dozens of hostages Saturday at a Trader Joe’s in the Silver Lake neighborhood of Los Angeles. Autopsy confirms homicide in care home worker’s death<\/h3>\n

Joplin, MO — An autopsy performed today on the body of Joplin care home employee Monte Little Jr.<\/strong> has determined that Little’s death was a homicide<\/a>. Little, 19, was slain Monday night at The Arc of the Ozarks care home at 2809 N. Santa Fe Drive in an apparent altercation with a co-worker. An autopsy conducted today at Frontier Forensics in Kansas City, Kansas, determined the death was a homicide. But police have not as yet released what the autopsy found to be the cause of death.<\/p>\n

Construction worker electrocuted at Thompson High School<\/h3>\n

ALABASTER, AL (WBRC) –\u00a0<\/span>A construction worker working on electrical work inside the Thompson High School Athletic Center was electrocuted Tuesday afternoon<\/a>. He was taken to a local hospital where authorities pronounced him dead. Witnesses tell authorities that the victim was working on a ladder inside the new athletic facility.\u00a0Construction at the school has been stopped for the rest of the day.\u00a0 The Shelby County Coroner’s Office identified the victim as 35-year-old Ladarius Williams<\/strong>.<\/p>\n

9\/11 losses continue: Honoring White Plains Fire Department Lt. Clark Douglass<\/h3>\n

White Plains, NY — The death of retired White Plains Fire Lt.\u00a0Clark Douglass<\/strong> brings another 9\/11 loss<\/a> for our communities. And sadly, there will be more to come. Douglass died on July 21. He was 53. Douglass had joined in the rescue and recovery efforts at Ground Zero, one of about 60 White Plains firefighters who headed to the World Trade Center site to help their\u00a0FDNY brethren.\u00a0 Douglass retired in March as a direct result of his 9\/11 illness.<\/p>\n

COMPANY COMMENTS ON FOREST CITY MAN KILLED IN FORT DODGE CONSTRUCTION ACCIDENT<\/i><\/h3>\n

FOREST CITY, Iowa \u2013 A Forest City man working as a contractor was killed at a manufacturing plant<\/a> in Fort Dodge. Shannon Torkelson,<\/strong> 41, died as a result of the construction accident when he was hit and pinned under a steel beam Thursday evening. Iowa Occupational Safety and Health Administration spokeswoman Deborah Babb says the beam was being erected as part of a construction project at CJ Bio America in Fort Dodge when the accident happened.<\/p>\n

Man killed in tractor accident at Monterey Mushrooms.<\/h3>\n

Royal Oaks, CA — A 60-year-old man from Aromas was killed on Monday morning after being run over by a tractor while working<\/a> at Monterey Mushrooms in Royal Oaks. According to a press release from California Highway Patrol, around 11am on July 23, Mark Odle<\/strong> was working at the facility when he was hit and run over by a tractor. Medics arrived and pronounced Odle dead.\u00a0 This is the second incident involving Monterey Mushroom after the\u00a0District Attorneys of Monterey, Santa Cruz and San Luis Obispo counties\u00a0reached a settlement with\u00a0the business, and its subsidiary, Spawn Mate, over alleged violations of the\u00a0California Fish and Game Code. The company agreed to pay more than $300,000 in penalties.<\/p>\n

Lawn care worker’s death was accidental<\/h3>\n

GOSHEN, IN \u2014 An Elkhart woman apparently choked to death during a lawn maintenance job<\/a> at Roxbury Park mobile home community in Goshen Monday.\u00a0Syma DiCarlo<\/strong>, 46, died from traumatic asphyxia, and the death was an accident, the Elkhart County Coroner reported as preliminary findings following an autopsy Tuesday.\u00a0A co-worker found DiCarlo unconscious and not breathing under a riding lawn mower at 357 Marlin Court around 11:30 a.m. Monday, Goshen police said. Information about what led to DiCarlo\u2019s death was not provided Tuesday. The case remains under investigation.<\/p>\n

According to a press release from the Guilford Police Department, on July 19 at around 1:50 p.m. Guilford Police and Fire personnel were dispatched to the area of State Street and Little Meadow Road for a reported work site incident.<\/p>\n

Gas tank explosion in Kemah leads to welder’s death<\/h3>\n

Kemah,\u00a0 TX — A welder is dead after an explosion<\/a> on a boat he was working on Monday morning near Kemah. The man, 57, was welding a small gas tank on an off-shore boat, which was dry-docked at a business in the 900 block of Lawrence Road, when deputies believe the fumes from the tank ignited, according to the Galveston County Sheriff’s Office Chief Deputy Darrell Isaacks. The man was rushed to Clear Lake Regional Medical Center, where he was pronounced dead upon arrival, Isaacks said.\u00a0The business where the explosion occurred, Gulf Coast Trawl Door, specializes in boat welding and fabrication.<\/p>\n

Concord, MA — A 46-year-old man died Wednesday as a result of an industrial accident<\/a> at a hospital in Concord, Middlesex District Attorney Marian Ryan said in a statement. The man died while working in a crawl space on a boiler system at Emerson Hospital, according to a preliminary investigation by State Police assigned to the district attorney\u2019s office, the Concord police and fire departments, and the US Occupational Safety and Health Administration, the statement said.<\/p>\n

Name of worker killed in Letterkenny explosion released<\/h3>\n

CHAMBERSBURG, Pa. \u2014 The name of the worker killed following Thursday’s explosion<\/a> at the Letterkenny Army Depot was released Saturday evening through a post on the depot’s Facebook page.\u00a0Eric S. Byers<\/strong>, 29, of Saltillo, in Huntingdon County, Pa., died Thursday at Lehigh Valley Hospital in Allentown, Pa., as a result of injuries he sustained at the facility north of Chambersburg.\u00a0 Letterkenny representatives were not immediately available for comment Saturday, but as of Friday morning, two employees remained hospitalized in critical but stable condition, while the others were treated at Chambersburg Hospital and released, according to Col. The arborist was identified as Luis Garcia<\/strong>, 48, of Santa Rosa. He was the sole owner of Ponce Tree Removal Service, a private tree-cutting company, according to a spokesman for Cal-OSHA. Two employees ran to help Garcia after he was struck by the tree Wednesday afternoon in the 1300 block of West Dry Creek Road. They administered CPR but could not revive him, sheriff\u2019s officials said in a statement.<\/p>\n

Investigation continues into fatal Covington construction incident<\/h3>\n

Covington, KY — Covington police have still not released the name of a construction worker killed in an incident<\/a> Thursday. The death happened\u00a0near Tuscany Drive and Holds Branch Road in Covington, where excavation was underway for a Fischer Homes project.\u00a0 Covington police spokesman Col. Brian Steffan said a contractor\u00a0was killed when the heavy machinery\u00a0he was operating fell from the top of a hill, rolling several times and landing\u00a0on top of the man. He was pronounced dead at the scene. Steffan said the name is being withheld as next of kin are being notified.<\/p>\n

The victim was identified by Kernersville police as Jeffrey Whitworth<\/strong> of Kernersville. \u201cThe deceased was operating a forklift and was in the back of a tractor-trailer,\u201d Jones said. The tractor-trailer had been backed up to a concrete dock, but at some point the tractor-trailer was moved away from the dock, leaving a gap, Jones said. Jones said that as Whitworth backed the forklift out of the tractor-trailer, it fell 4-5 feet to the ground. Whitworth was crushed.<\/p>\n

Allen was a new employee for South Dakota-based Rushmore Roofing, said David Darnell, company owner. \u201cHe hadn\u2019t been there very long. Officials say 30-year-old Wilber Dimas<\/strong> died Sunday, more than two weeks after the June 26 explosion at Coryell Memorial Hospital in Gatesville. Dimas was working at a construction site at the hospital when the blast occurred.\u00a0 More than a dozen workers were hurt.
