[Source: Washington Department of Labor and Industries]<\/p>\n<h3 class=\"elementor-heading-title elementor-size-default\">Georgia police officer killed in shooting<\/h3>\n<p class=\"elementor-heading-title elementor-size-default\">SNELLVILLE, Georgia (AP) \u2014 A Georgia police officer has been <a href=\"https:\/\/kwwl.com\/news\/national-news-from-the-associated-press\/2018\/10\/20\/georgia-police-officer-killed-in-shooting\/\">killed after being shot<\/a> while responding to a suspicious vehicle parked near a middle school. Police say Gwinnett County police officer <strong style=\"font-size: 16px;\">Antwan Toney<\/strong><span style=\"font-size: 16px;\"> died at a hospital. The 30-year-old had been with the department for nearly three years. The shooting happened Saturday about 3 p.m. near Shiloh Middle School. Authorities say the suspects crashed the vehicle a short time later and witnesses reported seeing as many as four people running from the scene. It\u2019s unclear if any of the suspects were injured.<\/span><\/p>\n<h3 class=\"headline\">Construction worker killed by falling wood at GR development<\/h3>\n<p>GRAND RAPIDS, Mich. (WOOD) \u2014 A construction worker was <a href=\"https:\/\/www.woodtv.com\/news\/grand-rapids\/construction-worker-killed-by-falling-wood-at-gr-development\/1538139868\">killed Saturday after a piece of wood flew free from a building<\/a> being worked on in Grand Rapids. It happened around 2 p.m. at 50 Monroe Ave. NW, where crews are working to restore the buildings to their\u00a0original appearance. Upon completion, there the\u00a0buildings\u00a0will be anchored by Townsquare media and its radio stations and a Marriott\u00a0hotel\u00a0run by Amway Corporation. Police said a piece of wood came free, striking and killing a worker on the ground inside the building. The worker was taken to St. Mary&#8217;s Hospital, where he was pronounced dead.<\/p>\n<h3 class=\"headline\">Tree trimmer killed in 60-foot fall after boom bucket broke<\/h3>\n<p>HOLTON, Kan. (KSNT) &#8211; HOLTON, Kan. (KSNT) &#8211; Jackson County Sheriff Tim Morse says moist ground may have been a factor in Wednesday&#8217;s <a href=\"https:\/\/www.ksnt.com\/news\/local-news\/wet-ground-may-have-caused-tragic-holton-crane-accident\/1531486973\">tragic crane crash<\/a>. One man was killed, another seriously injured at a house in south Holton.\u00a0 Initially both men were rushed to the hospital in critical condition. However Jackson County Sheriff Tim Morse said <strong>John Zibell<\/strong>, 66, from Holton was later pronounced dead at the Holton hospital. The other man, now identified as 29 year-old\u00a0Zachary Estrada,\u00a0 of Holton, sustained an amputation to his right arm and was transported to the University of Kansas Medical Center in Kansas City, Kansas. Both men were thrown from a lift basket which was about 30 feet in the air, after the mobile crane carrying them overturned. The men were working on the three-story house&#8217;s roof at the time of the accident. The Sheriff said The men had been working from a large basket or work platform that was attached to the boom of the T-Rex forklift.<\/p>\n<h3 class=\"headline\">Florida firefighter dies while clearing Hurricane Michael debris<\/h3>\n<p>GULF COUNTY, Fla.\u00a0&#8211; A Florida firefighter <a href=\"https:\/\/www.wsoctv.com\/news\/trending-now\/florida-firefighter-dies-while-clearing-hurricane-michael-debris\/856561936\">died Thursday after a tree fell on his tractor<\/a> while he was helping family members to clear debris left by Hurricane Michael, officials with the Gulf County Sheriff\u2019s Office said.\u00a0Deputies said Gulf County Fire Coordinator<strong> Brad Price<\/strong>, 49, of Wewahitchka, died at the scene. Price worked as a firefighter and paramedic in Bay County and previously worked for Lynn Haven Fire and Emergency Services.<\/p>\n<h3 id=\"articleHeadline\" class=\"p-name\">City supervisor planting flowers struck, killed on Center Point Parkway median<\/h3>\n<p class=\"paragraph\">Birmingham, AL &#8212; A Center Point city worker was <a href=\"https:\/\/www.al.com\/news\/birmingham\/2018\/10\/city-worker-planting-flowers-struck-killed-on-center-point-parkway-median.html\">killed Thursday when she was struck by a car<\/a>. The crash happened at 11:15 a.m. in the 1500 block of Center Point Parkway. Chief Deputy Randy Christian said deputies responded to find the woman critically injured. She was pronounced dead at the scene. Center Point Mayor Tom Henderson said the victim was a city horticulturist who was planting flowers when she was struck. Authorities said it appears the victim was kneeling in the median when the driver of a small SUV apparently veered into the median and struck the woman. before crashing into two Center Point city work trucks that were unoccupied at the time. The driver was not injured.<\/p>\n<h3 class=\"headline\">Worker killed in machine accident at West Virginia coal mine<\/h3>\n<p>CHARLESTON, W.Va. (AP) &#8211; West Virginia regulators said\u00a0a coal mine employee\u00a0<a href=\"https:\/\/www.wboy.com\/news\/worker-killed-in-machine-accident-at-west-virginia-coal-mine\/1533211714\">died in a machinery accident<\/a>.\u00a0 The Office of Miners&#8217; Health, Safety and Training said\u00a0<strong>Roger W. Herndon<\/strong>, 33, of Rainelle was struck in the side by a piece of steel auger drill.\u00a0 The accident occurred Wednesday at the Princess Polly Anna and JCT Enterprises surface mine in Greenbrier County. Herndon\u00a0was pronounced dead at a hospital.\u00a0 Herndon was an auger helper at the mine.<\/p>\n<h3 class=\"hero-heading flex-media-heading heading-larger heading-largest-for-medium-up text-tighter block-normal\">Northern Colorado Man Dies in Workplace Accident<\/h3>\n<\/div>\n<div class=\"article-body-item ad-in-text-target \">\n<p>JOHNSTOWN, Colo. Weld County officials say a piece of wood thrown from a machine at Sun Mountain Custom Doors struck 54-year-old <strong>Steven Verle Deniston<\/strong> in the chest on Tuesday. The Milliken man was pronounced dead at the hospital in nearby Loveland.<\/p>\n<h3 class=\"entry-title\">CHEVRON WORKER DIES AFTER FALLING<\/h3>\n<p>Pascagoula, Mississippi &#8212; The contractor at Chevron in Pascagoula who died last night <a href=\"https:\/\/www.wxxv25.com\/2018\/10\/17\/chevron-worker-dies-falling\/\">after falling on the job<\/a> has been identified. Albert Lee Byrd the Second, an employee of Performance Contractor\u2019s Inc., received fatal injuries after falling from an elevated structure at the Pascagoula Refinery. The limited details were released in a statement from Chevron. Page<\/strong>, 41, was pronounced dead on the scene Monday by McKean County Coroner Mike Cahill. Company spokesman Rick Kimble said the accident that led to Page\u2019s death is still under investigation.<\/p>\n<h3 class=\"headline entry-title\">Worker Killed On Racetrack Remembered As &#8216;Sweet Man&#8217;<\/h3>\n<p>Monterey County, CA &#8212; The Monterey County Sheriff&#8217;s Office has identified a track worker who died at WeatherTech Raceway Laguna Seca on Sunday as<strong> Edward Reins,<\/strong> 68, from Moss Landing.\u00a0 Reins was regulating the track with flags as a corner worker when he left his turn station and was hit by at least one racecar in the driving area, according to the race organizer, Exclusive Track Days.\u00a0 Organizers do not know why Reins entered the driving area, but said there was oil on the track due to a racecar experiencing technical difficulties and several participants lost traction in the area.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"elementor-widget-container\">\n<h3 class=\"headline\">Saint David firefighter dies during training<\/h3>\n<p>ST. DAVID, AZ &#8211; A Saint David Fire District firefighter <a href=\"https:\/\/www.abc15.com\/news\/region-central-southern-az\/tucson\/saint-david-firefighter-dies-during-training\">died in training<\/a> Wednesday. According to Saint David Fire District, 36-year-old <strong>Joshua Eugin<\/strong>\u00a0died during a multi-department training exercise. Eugin was taking part in a &#8220;blackout drill,&#8221; in which firefighters simulate rescuing victims inside a dark structure with fake smoke.\u00a0\u00a0Eugin collapsed at 8 p.m. EMTs\u00a0treated him with CPR while a paramedic unit made its way to the scene. He was transported to the Benson Hospital emergency room. Eugin was pronounced dead at 10 p.m.<\/p>\n<h3 class=\"headline\">Cody man dies on Chief Joseph project<\/h3>\n<div class=\"meta\">\n<div class=\"subscriber-preview\">\n<p>Cody, WY &#8212; A Cody man was killed Friday after <a href=\"http:\/\/www.codyenterprise.com\/news\/local\/article_1039dcc0-d24d-11e8-8846-274f9c724227.html\">his truck ran off a 250-foot cliff<\/a> while working on the Chief Joseph Slide repair project. <strong>Ronald Frankenberry<\/strong>, 26, was working as a truck driver for Oftedal Construction, a contractor on the Wyoming Department of Transportation project, according to WYDOT. Wyoming Highway Patrol and Wyoming Occupational Safety and Health Administration have investigated the incident, with OSHA clearing the worksite Tuesday, allowing the construction crew to start work again Wednesday morning.\u00a0 The accident happened after 9 p.m. Friday \u2013 crews have been working back-to-back 10-hour shifts each day to finish the repairs \u2013 when the truck driven by Frankenberry side swiped another haul truck. Pence said Frankenberry then continued down the hill, across the berm, out through the trees and vaulted over the edge of the cliff. The truck came to a rest in Dead Indian Creek below. Frankenberry was wearing his seat belt, Pence said, but was nonetheless ejected out the back window by the force of the impact and was declared dead on scene.<\/p>\n<div id=\"story-header\">\n<h3 class=\"title\">Construction company investigated after death of employee<span id=\"update_date\"><\/span><\/h3>\n<\/div>\n<div id=\"story-body-items\">\n<div class=\"dateline-storybody\"><span class=\"dateline\">ROCKINGHAM, VT. &#8212;\u00a0<\/span>Federal officials are investigating a Vermont roofing and construction company <a href=\"https:\/\/www.thestate.com\/news\/business\/national-business\/article220154895.html\">whose employee died in a fall<\/a> at a job site in New Hampshire. The Brattleboro Reformer reports the Occupational Safety and Health Administration opened an investigation into Jancewicz &amp; Son of Bellows Falls after 33-year-old <strong>Derek Goldsmith<\/strong> died Sept. 22. OSHA representative Edmund Fitzgerald says in an email the investigation is ongoing. Goldsmith was a crew manager for Jancewicz &amp; Son, whose corporate name is Home Improvement Company of Vermont, Inc.<\/div>\n<\/div>\n<div>\n<h3 class=\"headline\">Truck driver killed in Turnpike crash<\/h3>\n<div class=\"meta\">\n<p>West Providence Township, PA &#8212; A <a href=\"https:\/\/www.tribdem.com\/news\/truck-driver-killed-in-turnpike-crash\/article_cee77c2a-d281-11e8-a9db-0baa93b6de6f.html\">fatal tractor-trailer accident<\/a> occurred along the Pennsylvania Turnpike in West Providence Township, Bedford County on Wednesday. It took place in the eastbound lane near Mile Marker 158 shortly before noon. The driver of the truck attempted to pass another trailer but failed to maintain the lane, causing a collision, according to a report provided by the Pennsylvania State Police. The truck that failed to properly navigate the pass crashed through a guide rail, traveled down an embankment, struck a culvert and caught fire.\u00a0The operator of the vehicle, who was pronounced dead at the scene, could not be immediately identified. \u201cI can\u2019t even say if it was a \u2018he,\u2019 because the deceased is burned that bad,\u201d Bedford County Coroner Rusty Styer said.<\/p>\n<h3 class=\"headline\">Man dies after tractor rolls on him in northwest Iowa<\/h3>\n<div class=\"meta\">\n<p>INWOOD, IA \u2014 Authorities say a man <a href=\"http:\/\/www.clintonherald.com\/news\/local_news\/man-dies-after-tractor-rolls-on-him-in-northwest-iowa\/article_2b228dd1-a985-5d39-9e92-0014a77ff060.html\">died in a northwest Iowa farm accident<\/a>. The Lyon County Sheriff&#8217;s Office says the accident occurred Friday on a farm northwest of Inwood. The office says 59-year-old <strong>Paul DeBoer<\/strong> was operating the machine when it rolled over and pinned him. DeBoer was pronounced dead at the scene.<\/p>\n<h3>Driver, cattle killed in crash on I-84; cows get loose, closing 100 miles of interstate<\/h3>\n<p>BAKER CITY, Ore. &#8211; A truck driver and several cattle <a href=\"https:\/\/katu.com\/news\/local\/traffic-barely-moo-ving-in-ne-oregon-after-truck-hauling-cattle-crashes-on-i-84\">died when a semi hit a guardrail<\/a> and overturned between Baker City and La Grande on Tuesday afternoon. Oregon State Police said the driver died at the scene. The driver&#8217;s identity is not yet known. &#8220;Several cattle escaped and are loose on the freeway,&#8221; state police said on Twitter, &#8220;unfortunately several cattle also died in the crash.&#8221;<\/p>\n<h3 class=\"entry-title\">North Carolina trooper killed in traffic stop\u00a0shooting<\/h3>\n<p>COLUMBUS COUNTY, N.C. \u2014 A North Carolina state trooper was <a href=\"https:\/\/fox59.com\/2018\/10\/17\/north-carolina-trooper-killed-in-traffic-stop-shooting\/\">fatally shot<\/a> early Wednesday during a traffic stop in Columbus County, and a suspect is in police custody, authorities said. <strong>Trooper Kevin K. Conner <\/strong>died after being shot around 12:15 a.m. near the community of Williams, a few miles from the South Carolina state line, the North Carolina Department of Public Safety said. The incident began when Conner pulled over a driver of a pickup truck for speeding. The driver fired several shots, hitting the trooper, who died after being transported to a hospital, the department said.<\/p>\n<h3>Man dies after falling off roof in Moon<\/h3>\n<p class=\"News-body-text\">Moon, PA &#8212; A 24-year-old man died at 9:26 a.m. Tuesday after <a href=\"https:\/\/triblive.com\/home\/contactus\/\">falling off a roof<\/a> in Moon. <strong>Oscar Trujillo<\/strong> of the St. Louis suburb of Fairmont City, Ill., died at Heritage Valley Sewickley hospital, according to the Allegheny County Medical Examiner\u2019s Office. Trujillo fell off a roof at 125 Apple Blossom Way at 8:48 a.m., the Medical Examiner\u2019s Office said. Trujillo was working on a roof at the main building of what will be the Apple Blossom Senior Living complex, Moon police Chief Greg Seamon said. \u201cIt was a frosty morning. He went up on the roof and slipped.
