John Ditterline<\/strong>, 55, of Equality, Illinois, was pronounced dead at 4:45 a.m. Saturday. Hamilton County Coroner Steve Bowling said Ditterline was working for Clay, Kentucky-based S & L Industries as a contract employee in the underground mine when he was killed. Workers found Ditterline’s body near a set of 18 feet wide doors that close automatically, Bowling said.<\/p>\n

L Brands employee dies after being crushed between conveyor belt, boxes<\/h3>\n

REYNOLDSBURG, Ohio (WCMH) — An L Brands, Bath and Body Works employee died after he was crushed<\/a>, caught between boxes and a conveyer belt on January 4th, a spokesperson for the Occupational Safety and Health Administration (OSHA) said Monday. According to OSHA spokesperson Larry Johnson, the incident happened around 1:45 a.m. on Jan. 4th at 7 Limited Parkway. Employees were loading product onto a truck using an expandable conveyor when the employee, identified as 56-year-old Harvey Beavers<\/strong> in the incident report, got pinned between it and boxes, and was crushed. Beavers was pinned in the machinery for about 10 minutes before he was extricated. CPR was performed and he was transported to the hospital, where he was later pronounced dead. This is the third fatal industrial accident in as many weeks in Central Ohio after a man was\u00a0crushed to death at Liberty Casting in Delaware, Ohio<\/a>\u00a0and an\u00a0employee was killed at the Fluvitex\u00a0facility.<\/a><\/p>\n

2 men killed in industrial accident at Cantua Creek ranch in Fresno County<\/h3>\n

FRESNO, Calif. (KFSN) — Two men were killed after being caught under a tree nut shaker<\/a> at a ranch in Cantua Creek in Fresno County.\u00a0\u00a0Fresno County Fire responded to the incident just around 11:20 a.m. on Saturday, where they found a 66-year-old man and a 31-year-old man had been crushed while trying to make a repair on the machine.\u00a0\u00a0According to deputies, the workers were trying to disconnect hydraulic hoses under the shaker when the arm fell. Both men were pronounced dead when paramedics arrived.\u00a0“The machine shaker arms weren’t functioning properly<\/a> ad one of the workers had gone under the machine to fix the hoses for the hydraulic lines and the arms fell down and crushed them,” Lieutenant Devins said.\u00a0Family members have identified one of the victims as 66-year-old Arnold Torres<\/strong>.<\/p>\n<\/div>\n

The death has been ruled accidental. Tow Truck Driver Killed in Hit-And-Run on Grand Prairie Interstate<\/h3>\n

Dallas, TX – The son of a tow truck driver struck and killed<\/a> in a hit-and-run crash as he helped a woman stranded on Interstate 20 early Friday morning is asking for help finding the driver that killed his father. According to Joshua Mosby, his cousin, 46-year-old Keith Holt<\/strong>, said he was headed out to help a client who was stuck on the highway with her child. While helping that woman in the westbound lanes of Interstate 20, near the South Great Southwest Parkway overpass, Holt was fatally struck by a passing vehicle.\u00a0 The driver, believed to be behind the wheel of a 2012-2015 white or multi-colored Nissan Altima, kept going. Holt died at the scene.<\/p>\n

Log truck driver killed in Counce crash<\/h3>\n

Counce, TN — The driver of a tractor-trailer hauling logs was killed Thursday afternoon in a single-vehicle crash<\/a> on Tenn. 57 in Counce just west of the railroad tracks. The Tennessee Highway Patrol said Stacey L. Ballard<\/strong>, 40, of Sardis, Tennessee, was driving east on the highway when the 18-wheeler veered off the right side of the roadway and overturned.
\nAlcohol and drugs were not cited as factors and Ballard was wearing a seatbelt, the THP said.<\/p>\n

OSKALOOSA, Kan. | Jefferson County authorities said a waste services employee died at a landfill north of Lawrence. Sheriff Jeff Herrig said the man, who died Friday morning, was an employee of Hamm Waste Services in Jefferson County. The victim\u2019s name has not been released. Herrig said the man was crushed behind a semi-trailer that was being backed up to a machine used to empty large amounts of waste. The Kansas Highway Patrol is investigating.<\/p>\n

Man crushed to death at Liberty Casting had been injured on the job before, family says<\/h3>\n

DELAWARE, Ohio (WCMH) — Michael Crow<\/strong>, 72, was killed Friday morning<\/a> when\u00a0a piece of equipment crushed him while working at Liberty Casting Friday morning. His family tells NBC4 it wasn’t the first injury he had received on the job. NBC4 obtained records from the explosion at the iron foundry\u00a0in August 22, 2018, which burned several workers with molten iron. One of the victims\u00a0died from their injuries, Larry Johnson with OSHA said. Those records show Liberty Casting received multiple violations and was fined more than $70,000\u00a0as a result. Just over four months later, Michael Crow\u00a0died in an unrelated industrial accident at the foundry.<\/p>\n

Worker Crushed In North Hollywood Identified<\/h3>\n

NORTH HOLLYWOOD, CA \u2014 A worker killed on New Year’s Eve when a car was dropped on him<\/a> at a North Hollywood facility was identified Wednesday by the coroner’s office. Sixty-three-year-old Bruce Runyon<\/strong>, of Winnetka, was crushed to death when a car that was raised on a floor jack fell on him, according to coroner’s Lt. Larry Dietz. Runyon was trapped underneath the vehicle, and firefighters worked to free him, but they couldn’t save him, accoding to the Los Angeles Fire Department.<\/p>\n

Las Vegas, NV — – Las Vegas police are searching for a woman accused of using a stolen car to run over and kill a nail salon worker<\/a> after failing to pay for a $35 manicure. The Clark County Coroner’s Office identified the manicurist as 51-year-old Ngoc Q. Nguyen<\/strong> of Garden Grove, California, and said the cause of death was multiple blunt-force injuries.\u00a0The woman got a manicure Saturday at a salon about a mile and a half from the Las Vegas Strip, but her credit card was declined when she went to pay.The woman went to her car, telling salon workers she would come back to pay another way but then tried to drive away, according to police Lt. Ray Spencer. The manicurist ran in front of the car to try to stop the woman from driving off but was hit by the vehicle, police said.<\/p>\n

The man got out of his car and called out for help. A nearby resident called 911.The man was taken to the hospital. His age, identity and condition have not been released.<\/p>\n

Chicago, IL — Authorities have identified a roofing worker was killed when a fire broke out<\/a> as they worked on top of a River North parking garage Saturday afternoon, seriously injuring four others, according to the Chicago Fire Department.\u00a0Terrell Webster<\/strong>, a 47-year-old construction worker, was found dead at the scene, according to fire officials and the Cook County Medical Examiner\u2019s Office. 2 Weeks Into Her Job, Intern Killed By Lion At North Carolina Sanctuary<\/h3>\n

Caswell County, NC — A lion at a North Carolina private wildlife sanctuary fatally mauled a 22-year-old worker<\/a> on Sunday before being shot and killed by local law enforcement, officials said. The sheriff’s office in Caswell County, North Carolina, said Alexandra Black<\/strong>, who started her job as an intern at the facility two weeks ago, was part of a team doing routine cleaning of an enclosure when she was attacked, local media reported. Winlock Man Dies in Accident at TransAlta Coal Mine Pond<\/h3>\n

Centralia, WA — An employee working on a project to recover coal from a pond at TransAlta\u2019s Centralia mine site Saturday night was killed after a barge used in the dredging operation sunk<\/a>, according to the Lewis County Sheriff\u2019s Office. The Lewis County Coroner\u2019s Office identified the man Monday morning as Caleb D. Brown<\/strong>, 28, of Winlock. A n employee of a contractor was working on a 72-foot barge in a pond in an effort to dredge the pond to reclaim coal from the mud, according to the sheriff\u2019s office.\u00a0A supervisor reported last having contact with the employee at 7:30 p.m. At 7:45 p.m., employees realized the dredging operation had stopped and the power to the barge was shut off. The barge was held in place by a cable and powered by a high-voltage line.\u00a0Employees then discovered the barge had sunk and was under about 20-feet of water. They were unable to locate the employee working on the barge.<\/p>\n

He knew many residents by name. Their dogs, too.\u00a0Randy Irwin<\/strong>, a more than 28-year veteran of the U.S. Postal Service who worked out of the Wadsworth post office, was killed Wednesday afternoon when a minivan hit his mail truck<\/a> on state Route 94. He was pulling back into traffic about 4 p.m. when his truck was struck from the rear by a 2016 Dodge Caravan driven by Danielle M. Ullman on Route 94 north of Eastern Road, according to a news release from the Ohio Highway Patrol. The truck overturned and Irwin, who was not wearing a seat belt, was partially ejected.<\/p>\n

OSHA Inquiry Into Ortiz Death May Take Six Months;<\/h3>\n

Haverhill, MA — The federally-mandated investigation into the workplace death<\/a> of Haverhill roofer Carlos Ortiz<\/strong> may take up to six months to complete, a spokesman for the Occupational Safety and Health Administration tells WHAV. Employed by River Street\u2019s Progressive Roofing at the time of his death, Ortiz was working at 98 Essex St., on a roof nine stories up when he fell though what is believed to be an elevator shaft on Dec. 18. As WHAV previously reported, the job site was immediately shut down by Haverhill\u2019s Director of Health and Inspectional Services Richard MacDonald after police were called to the scene at 10:20 a.m., but contractors have since resumed work at the location of Ortiz\u2019s death.<\/p>\n

MIZPAH, N.J. \u2014 A young volunteer firefighter from New Jersey was killed Christmas morning when her car crashed into a guardrail<\/a> as she responded to an emergency call. Natalie Dempsey<\/strong>, 21, worked for the Mizpah Volunteer Fire Company 18-2 and was the vice president of the organization. The company announced her death on its Facebook page at 8:30 a.m. Tuesday.<\/p>\n<\/div>\n

Keota man dies from injuries after accident at Kalona Tire<\/h3>\n

KEOTA, Iowa (KCRG) –<\/span>A Keota man has died from injuries received in a workplace accident<\/a> that happened December 13.\u00a0Washington County officials say the accident happened around noon at Kalona Tire when a tire blew up. 27-year-old Cameron White<\/strong> was injured and emergency crews took him to University of Iowa Hospitals and Clinics. He died Tuesday.<\/p>\n

Westerly DPW mechanic killed in accident at town garage<\/h3>\n

WESTERLY, RI \u2014 The chief mechanic for Westerly\u2019s Department of Public Works, described as a happy, well-liked man, was fatally injured Wednesday afternoon while bending to look under a truck<\/a> that he had just decided to pull from service for a brake problem.\u00a0Angelo Turano<\/strong>, 53, was knocked down and partially run over by a rear wheel of the 40-foot trailer, which crushed his shoulder and chest.\u00a0 The truck, used for hauling trash from the transfer station to the state landfill in Johnston, pulled in from the entrance closest to Route 91, drove behind the garage and pulled up alongside the buildingd. The driver, Scott A. Wells, parked the rig and walked around to the passenger side to tell Turano about a problem with the tractor-trailer\u2019s air brakes. They agreed that Wells would park the truck elsewhere to await repairs, and Wells walked back to the driver\u2019s side and climbed into the cab. Turano and another DPW worker, Robert Pelchat, on the passenger side of the trailer, were not visible to Wells, who didn\u2019t see that Turano had bent under the trailer to look at the brakes. Wells pulled forward, moving only about two feet, before Pelchat yelled for him to stop. One of the trailer\u2019s rear tires knocked Turano onto his back and rolled over his left shoulder and chest.<\/p>\n

Neighbors shocked after employee killed in attempted armed robbery on Oakwood Avenue<\/h3>\n

SYRACUSE, N.Y. \u2014\u00a0<\/span>86-year-old Clara Hill was asleep in her home when she was startled awake. “I heard bullets and I got up and went to the living room window and I saw somebody laying over there right there,\u201d Hill said. Hill is stunned after learning an employee was killed<\/a> at Eddie’s Eagle Market just across from the street from here.\u00a0Police say he and the employee ended up in a fight, which spilled out onto the sidewalk before the shooting. Chartiers Township, PA — A West Virginia man who was burned in a Washington County gas plant explosion<\/a> has died, officials confirmed.\u00a0Jeffrey Fisher<\/strong>, 61, of Salem, W. Va. died Tuesday afternoon at UPMC Mercy hospital in Pittsburgh. Fisher was a burn victim at the hospital, according to the\u00a0Allegheny County Medical Examiner\u2019s Office, which did not release a cause of death. Three others were injured, but the company did not release any further details on their conditions. The blast\u00a0happened\u00a0December 13 at MarkWest\u2019s Houston Processing and Fractionation Facility in Chartiers Township. Officials from the company had said that the incident occurred near a pair of temporary storage tanks. Jeff Yates, Washington County\u2019s public safety director, said it is believed the workers were cleaning a pipeline<\/a> when vapors caught fire and ignited other combustible materials. Two victims were flown to Mercy and two to West Penn Hospital in Pittsburgh.<\/p>\n

The incident is under investigation by both the Baltimore Environmental Police and the Maryland Occupational Safety and Health Administration.\u00a0Raymond said Cruz was working for Archer Western, a company contracted by the city when he was killed;\u00a0 Three other Archer Western workers have been killed on the job<\/a> in Florida this year.\u00a0\u00a0Two men died in February working in a trench on Interstate 95 in Miami, where a concrete barrier fell on them. Paramedics were sent to the 2300 block of North Donatella Circle about 11:45 a.m. Monday, according to the Los Angeles Fire Department. Guarchaj Y Guarchaj<\/strong>, 23, died at the scene, according to the coroner\u2019s office. His hometown was not known. Guarchaj died of blunt force injuries he suffered in a fall from a roof, and his death was classified as an accident, the coroner\u2019s office reported.<\/p>\n

EDISON, N.J. \u2014 A gas station employee was shot and killed during a robbery<\/a> in New Jersey early Thursday, officials said. It happened at the Speedway Gas Station in Edison at about 4 a.m. Authorities said a man entered the gas station and held up the employee, John Bertram<\/strong>, with a handgun.
