State Police were called to the Cook Fence Company in Newport after they say a bundle of fence fell on top of a worker while he was unloading bundles from a trailer using a forklift. Sedita was taken to the hospital, where he died from his injuries.<\/p>\n<\/div>\n<h3 class=\"articleHeadline\">Officials piece together events before Ohio nursing home shooting<\/h3>\n<p>KIRKERSVILLE, Ohio &#8211; Authorities in Ohio are piecing together the events that led up to a <a href=\"http:\/\/www.cp24.com\/world\/officials-piece-together-events-before-ohio-nursing-home-shooting-1.3413917\">nursing home shooting <\/a>that left four people dead including the suspected gunman.\u00a0Kirkersville <strong>Police Chief Steven Eric DiSario<\/strong>, nurse <strong>Marlina Medrano<\/strong> and nurse&#8217;s aide <strong>Cindy Krantz<\/strong> were killed in the Friday attack. Suspect Thomas Hartless was found dead inside the Pine Kirk Care Center in Kirkersville, a village of some 500 residents, about 25 miles (39 kilometres) east of Columbus. Authorities said Stanislaus County Sheriff\u2019s <strong>Deputy Jason Garne<\/strong>r and his passenger, <strong>Officer Raschel Johnson<\/strong>, were responding to a burglary call when their car swerved and hit an auto wreckage business Saturday morning.<\/p>\n<h3 class=\"asset-headline\">Roofer dies after fall at apartment<\/h3>\n<p>WEST LAFAYETTE, IN \u2014 Thirty-three-year-old <strong>Alberto Dominguez<\/strong> died Thursday<a href=\"http:\/\/www.jconline.com\/story\/news\/local\/indiana\/2017\/05\/11\/roofer-killed-after-fall-apartment\/101554584\/\"> after a 20-foot fall<\/a> from the roof of a two-story\u00a0apartment building, Tippecanoe County Coroner Donna Avolt said. Dominguez, who is from Mexico but was living in Indianapolis, was part of a roofing crew working at 34 Country Squire Court, Avolt said.\u00a0Dominguez was working for an Indianapolis-based roofing company. He was wearing his safety harness. The accident occurred about 11:50 a.m. at the Mary Eaves apartment complex under construction in the 1200 block of West 16th Street.The men were working out of a wooden crate perched atop a hydraulic lift, loading doors into a third-story window, according to the Jacksonville Sheriff\u2019s Office. The pair fell when the crate they were using as a platform tipped over and came crashing to the ground, said Sgt. David Smith. It\u2019s unclear if the workers were strapped into harnesses. \u201cIt came loose from the lift and fell to the ground, taking them with it,\u201d Smith said.\u00a0The fall comes the same week as OSHA\u2019s National Safety Stand-Down to Prevent Falls in Construction. The initiative is marked by voluntary events for employers to educate and have dialogue with employees about workplace hazards, prevention methods and safety policies.<\/p>\n<h3>Tow truck driver struck while assisting motorist dies<\/h3>\n<div class=\"meta\">HARKER HEIGHTS, TX \u2014 A Killeen tow truck driver died Thursday morning after being <a href=\"http:\/\/kdhnews.com\/news\/local\/tow-truck-driver-killed-in-collision-on-us-highway\/article_ddf30298-364c-11e7-99fe-473a7477d461.html\">struck by a minivan <\/a>while attending to a vehicle on U.S. Highway 190\/Interstate 14. At approximately 6:20 a.m., the driver, who was a vehicle recovery specialist with Goodes Towing, was loading a vehicle onto the back of his truck when he was struck by the minivan driving westbound on 190, according to Harker Heights Police spokesman Sgt. Stephen Miller. Miller said the tow truck driver had activated his emergency headlights and was walking from the stalled vehicle back to the truck at the time of the collision. Wilkes County Chief Deputy Sheriff David Carson says the fire was reported early Thursday at the Countryside Golf Course in Elkin. Investigators say 64-year-old <strong>Gary Jordan<\/strong> died when a fire started in a building that stored golf carts. Investigators think the explosion occurred while Jordan was fueling the golf carts. About 20 carts were destroyed.Firefighters don&#8217;t know what caused the initial fire, but they think the fuel from the golf carts sparked the explosion.<\/p>\n<div class=\"subscriber-preview\">\n<h3>Worker dies after incident in flooded basement<\/h3>\n<p>Ketchum, ID &#8212; A Ketchum man <a href=\"http:\/\/www.mtexpress.com\/news\/blaine_county\/worker-dies-after-incident-in-flooded-basement\/article_9654d1fa-36a1-11e7-9436-0bd05542e7b0.html\">died Wednesday <\/a>after attempting to pump water from the basement of a home at the eastern end of Eagle Creek Road, north of Ketchum. \u00a0<strong>Mike Wirth<\/strong>, 54, who was hired to work at the site, died after being brought to the hospital Wednesday afternoon, family friend Susan Teren confirmed. Teren is also the office manager of Wirth Co., a Ketchum-based landscaping company that Wirth owned with his wife, Tami Nakaoka. The Ketchum and Sun Valley fire departments were dispatched around 11 a.m. Wednesday to reports of a man trapped in a flooded basement.<\/p>\n<\/div>\n<h3>Crash update: Semi-truck driver killed by detached container<\/h3>\n<p>Tacoma, WA &#8212; A semi-truck driver d<a href=\"http:\/\/mynorthwest.com\/629859\/two-crashes-blocking-both-directions-of-i-5\/\">ied after the container he was hauling detached <\/a>and smashed into the truck\u2019s cab. The driver, traveling on northbound I-5 in Tacoma, was braking for traffic when the truck hit a center Jersey barrier near South 72nd Street. We don\u2019t know at this point if he slipped and fell or it certainly could have been from another medical emergency at the time that led to his fall.\u201d Hoffman said Van Wyngarden received a significant injury to his head from the fall which is estimated to have been from about 10 to 12 feet. According to an AST dispatch posted online, 56-year-old <strong>James Schneider<\/strong> was an employee at the Togiak Fisheries Cannery near Twin Hills, Alaska. He was found lying face down in the mud at around 6:25 p.m.<\/p>\n<div class=\"teaser title1\">\n<h3>Norton Shores police officer killed in early morning crash<\/h3>\n<p>NORTON SHORES, MI \u2014 A single-car crash that <a href=\"http:\/\/www.grandhaventribune.com\/Law-Enforcement\/2017\/05\/10\/Norton-Shores-police-officer-killed-in-early-morning-crash\">killed a Norton Shores police officer<\/a> remains under investigation. <strong>Officer Jonathan Ginka<\/strong>, 34, was on duty when he crashed his patrol car into a tree shortly after 2:30 a.m. Wednesday, officials said.<\/p>\n<\/div>\n<h3>Two men, 37 and 83, die from unrelated falls in Jefferson County<\/h3>\n<p>Jefferson County, AL &#8212; Two men were killed in separate falls in Jefferson County this week. The Jefferson County Coroner&#8217;s Office on Wednesday identified the victims as<strong> Leon Barley Jr<\/strong>. and Archie Gordon McBride<strong>.<\/strong> Barley was 37, and McBride was 83. \u00a0The first accident happened about 10:30 a.m. Monday in Homewood. Chief Deputy Coroner Bill Yates said McBride fell off of a ladder at his Edgeknoll Lane home.\u00a0Hours later, <a href=\"http:\/\/www.al.com\/news\/birmingham\/index.ssf\/2017\/05\/two_men_37_and_83_die_from_unr.html\">Barley was killed in an industrial accident. <\/a>Authorities responded at 6:55 p.m. to the 1500 block of Sterlite Drive in Birmingham, north of Tarrant. He was taken to Albany Medical Center where he later died. The scene was so remote, crews could only access it by boat. The plane crashed just after 9 a.m. Monday near Lake Berryessa. The NTSB and FAA as well as the Napa County sheriffs have been investigating. The plane that crashed was a special amphibious plane that&#8217;s made nearby in Vacaville. The victims have been identified as 55-year-old <strong>Jon Murray Karkow<\/strong>, the pilot in command of the aircraft, and 41-year-old <strong>Cagri Sever<\/strong>, who was a passenger. Told the store does not sell singles \u2013 it is illegal \u2013 the man became angry, Singh said, sharing information provided by a second employee who was working that night.
