MEIGS COUNTY, Tenn. — A search for a missing Tennessee deputy came to an end Thursday night, hours after his patrol car was found in a river with another body inside, officials confirmed. Authorities announced the body of Meigs County Deputy Robert “R.J.” Leonard, who was previously reported missing, had been pulled from the Tennessee River around 6:30 p.m. local time. Leonard was last seen on Wednesday evening before responding to a call of a disturbance around the Birchwood Community, in the southern part of the state, The Tennessean reported. He made an arrest after arriving on the scene and was last heard from on his way to the jail. On Thursday, Leonard’s patrol car was found upside down in the Tennessee River, with the trunk blown open from the pressure of the water, District Attorney General Russell Johnson said at a press conference. The body of a woman was also found in the backseat covered in “a lot of mud,” Johnson said, adding she’s believed to be the person Leonard arrested Wednesday night but could not confirm her identity.

STERLING, Va. – A firefighter died when a house exploded in Loudoun County Friday night, and authorities say at least nine other first responders were injured in the blaze. Assistant Chief of Operations for Loudoun County Fire and Rescue James Williams told reporters in the Seneca Ridge community that the incident was first reported around 7:40 p.m. When fire crews arrived at the house on 347 Silver Ridge Drive, Assistant Chief Williams said that’s when the home exploded. “Multiple maydays were transmitted as the incident was upgraded to a multiple alarm fire,” he said. The assistant chief also mentioned that two civilians were injured, but he did not clarify if they were the homeowners.
Tree Trimmer Falls To His Death In North Twin Cities Metro
ANOKA COUNTY, MN — One man is dead after he fell from a tree in the north Twin Cities metro. On Tuesday at about 3:40 p.m., the Anoka County Sheriff’s Office and Allina ambulance responded to the scene on the 22400 block of Durant Street Northeast. Emergency responders found a man identified as Paul Eugene Malaske, 70, of East Bethel. He was declared dead at the scene. Investigators believe Malaske was on a ladder trimming a tree when he fell to the ground.
Coroner identifies 46-year-old man killed at UPS Worldport in Louisville
LOUISVILLE, Ky. — An employee is dead after an incident happened at the UPS Worldport facility in Louisville Thursday afternoon. According to authorities, members of the Louisville Muhammad Ali International Airport (SDF) Public Safety Department responded to an area at Worldport near the 900 block of Grade Lane around 4:30 p.m. on reports of an employee that had been struck by a vehicle. On Friday, the Jefferson County Coroner’s Office identified the victim as 46-year-old Dallas Carroll, of Georgetown, Indiana. Louisville Fire spokesperson Donovan Sims told WHAS11 News that a crew was dispatched to the facility for an “extrication involving a semi.” Police said Carroll died at the scene.

Muscle Shoals, AL — Muscle Shoals police say a man died Thursday in an apparent workplace accident. First responders were called to Southland Safes on Alabama 20 at about 4:49 p.m. Thursday. When police arrived, they found the body of a man in the business’ warehouse. Detectives and officers gathered evidence from the scene and determined that the man, whose name has not been released, had been the victim of a workplace accident.
Spring Grove man dies after being severely burned in industrial fire while working near O’Hare Airport
Elk Grove, IL — A Spring Grove man has died after he suffered severe burns while working at a business where an industrial fire broke out near O’Hare Airport. The Elk Grove Village Fire Department responded around 6:59 a.m. Tuesday to United Lane and Pan Am Boulevard in Elk Grove Village, which is just west of O’Hare International Airport. Elk Grove Village Acting Fire Chief Nathan Gac said crews received a call for a semi-truck on fire near a building. Firefighters were responding and noticed a large dark black smoke header, at which point they upgraded the alarm. Firefighters extinguished the fire in approximately 40 minutes and remained on the scene extinguishing hidden fires. Gac said a worker of the business, a semi-truck repair facility, suffered significant burn injuries. He was treated by paramedics and transported to Loyola University Medical Center in Maywood. The Cook County Medical Examiner’s Office said the worker died at the hospital late Tuesday evening. The office identified the victim Thursday morning as Oskar Dziadon, 50, of Spring Grove. A cause and manner of death is still pending.

Portage, AK — A state contract worker was killed Friday while working on a Seward Highway construction project near Portage, the Alaska Department of Transportation and Public Facilities said. The worker has not yet been identified. The incident occurred during work to remove a bridge at Twentymile River, the transportation department said in a statement. The bridge removal is part of a broader project to improve roads and bridges on the Seward Highway along Turnagain Arm south of Girdwood, between Mile 75 and 90.
2 Minn. police officers, 1 medic killed during domestic call identified
BURNSVILLE, Minn. — Burnsville city officials have identified the two police officers and paramedic who were shot and killed in Burnsville Sunday morning. WCCO identified the victims as Officers Paul Elmstrand, 27; and Matthew Ruge, 27; and Firefighter/Paramedic Adam Finseth, 40. The Minnesota Bureau of Criminal Apprehension reported the shooter was also dead and it is investigating the incident. The first responders were called to a home for reports of an armed man barricaded with family members. The situation escalated into gunfire, WCCO reported, and Elmstrand, Ruge and Finseth were killed, and another police officer was injured, but is expected to survive. Finseth was providing aid to an injured officer when he was fatally shot, police said. Minnesota Bureau of Criminal Apprehension Superintendent Drew Evans said during a news conference on Sunday that Finseth was a part of a SWAT team that had been called to a domestic situation.The suspect was reported dead and the family members were safely evacuated.
Richard Edwards Killed in Crash with Tractor Trailer on Cross Bronx Expwy near Jerome Ave
Bronx, NY – A driver lost his life after he was involved in a crash with a tractor-trailer in the Bronx. Reports concerning the collision show it happened on the Cross Bronx Expressway near Jerome Avenue. The wreck happened in the eastbound lanes just after 1:55 a.m. when Richard Edwards, 38, collided with the rear-end of a tractor-trailer for reasons still being investigated. The truck driver, 49, stayed at the scene and cooperated with authorities after the accident. Edwards was pronounced deceased at St. Barnabas Hospital after being transported from the crash scene by ambulance. The eastbound lanes of the Cross Bronx Expressway were closed at Jerome Avenue during the morning commute in order to allow for the investigation to be completed. Police are still working on figuring out how the accident happened.
Forklift driver killed at North Texas Walmart distribution center
FORT WORTH, Texas — A forklift driver died at a Walmart distribution center in Fort Worth on Friday morning, officials said. Fort Worth Fire Department officials said the driver ended up crushed between a forklift and a pallet rack at the distribution center, located at 15101 North Beach Street in far north Fort Worth. The incident happened around 6:15 a.m. Friday. Walmart officials in a statement said the company was “heartbroken over the passing of one our associates.” Walmart closed the facility for the day and was offering counseling “and other resources to our associates as they cope with this tragic event,” the statement said.

ELK GROVE VILLAGE, IL — A Spring Grove man, who was working at a semi-tractor repair facility near O’Hare International Airport that caught on fire Tuesday morning, has died from his injuries. Oskar Dziadon, 50, died at 10:19 p.m. Tuesday, at Loyola University Medical Center, according to the Cook County Medical Examiner. His identity was released Thursday. As of Friday morning, a cause of death was still pending. The incident occurred at the Pan Am Trucking facility at United Lane and Pan Am Boulevard in Elk Grove Village. Firefighters battled the blaze for several hours.

Indiana man identified after being hit by vehicle at UPS Worldport in Louisville
LOUISVILLE, Ky. (WAVE) – The Jefferson County Coroner released the identity of the man who died at the UPS Worldport facility in Louisville on Thursday. Dallas Carroll II of Georgetown, Indiana, died after was hit by a vehicle. He was 46 years old. Louisville Regional Airport Authority Police and Fire units responded to the facility on Grade Lane around 4:30 p.m., LMPD spokesperson John Bradley said. Carroll died before he could be taken to the hospital. Police believe no foul play was involved in Carroll’s death. The incident was reported to the Division of Occupational Safety and Health Compliance and an inspection has been opened.

Joey Jacobs says they received a call at about 10 a.m. reporting the industrial accident at the plant under construction at 2525 Highway 87. The Sheriff’s Office will investigate and turn over its information to OSHA. Yukiko Nakao, 29, of Port Arthur, was identified as the contract worker who was killed, Golden Triangle Polymers is a joint venture between Chevron Phillips Chemical and QatarEnergy.

Authorities say 65-year-old George Versak was pronounced dead around 9 a.m. Wednesday at Thompson Masonry Contracting located in the 800 block of Cross Street. Officials from the Montgomery County Coroner’s Office ruled the manner of death an accident and say Versak was struck by an industrial forklift.
Elderly man killed in truck yard accident, Laredo police say
LAREDO, Tex. (KGNS) – The Laredo Police Department is investigating an incident that claimed the life of a 72-year-old man. It happened around 8:44 p.m. on Monday, February 12, at a truck yard located in the 11900 block on Conly Road. The man was found unresponsive under a Ford F350 truck when authorities arrived. He was pronounced dead at the scene. The identity of the victim has been confirmed, although authorities have not released his name. The police are looking into what happened, and they’ve called in experts from the Occupational Safety and Health Administration (OSHA) to help figure out if there were any safety issues at play.
Sheridan Police Officer Killed In Line Of Duty
SHERIDAN, WY — Law enforcement and SWAT agencies from around the state were still in a standoff with a barricaded man as of 11 p.m. Tuesday evening, about 12 hours after the suspect reportedly killed a Sheridan Police Department officer. The suspect was still alive and walking around in his home with an AR rifle in one hand and a shotgun in the other, according to scanner traffic. Sgt. Nevada Krinkee was shot and killed Tuesday morning while trying to serve a trespass warning on a man near the intersection of 5th and Val Vista streets, SPD said in a statement.
Postal worker killed in crash that flipped vehicle on 103rd Street
JACKSONVILLE, Fla. – A 40-year-old postal worker died Tuesday after a multiple-vehicle crash on 103rd Street. The report from the Florida Highway Patrol said the postal worker was driving east on 103rd Street just before 4 p.m. When she attempted to make a left turn onto Lambing Avenue, a white SUV headed west in the left lane on 103rd Street hit her. The postal worker died at the scene. The 58-year-old woman driving the sedan and the 45-year-old woman driving the SUV, who were both wearing their seat belts, suffered minor injuries, the report stated. Family says the victim in the crash was 40-year-old Marquite Scurry. Scurry’s cousin said she was a “loving, caring” mother of five children. She loved to bake and cook, and was a hard worker.
Tow truck driver struck by passing vehicle along inbound lanes of US 290
HARRIS COUNTY, Texas — A tow truck driver was struck and killed by a vehicle along the Northwest Freeway on Tuesday, according to Harris County Sheriff Ed Gonzalez. According to Houston TranStar, it happened in the eastbound (inbound) lanes just inside Beltway 8. It was verified around 5:15 p.m. Gonzalez said the Harris County Sheriff’s Office Vehicular Crimes Division responded to the scene. According to Gonzalez, the tow truck driver, Simon Garcia, was loading a disabled vehicle onto his white 2013 Isuzu NRR flatbed wreck. Garcia’s wrecker was said to be stopped on the highway’s right lane, with Garcia standing outside the vehicle near the driver’s door. was out of his truck loading up another vehicle when he was hit. Initially, Gonzalez said, the tow truck driver was in fair condition, but while he was being taken to an area hospital for treatment, his condition quickly deteriorated. He said he was in critical condition. He was later pronounced dead, Gonzalez said.

Roanoke Rapids, NC — A railroad worker was killed on CSX train tracks in Roanoke Rapids after being hit by a construction train on Tuesday afternoon. Roanoke Rapids police, who are investigating the fatal workplace accident, said the CSX employee died around 1:30 p.m. on the company’s tracks near the 10th Street crossing. A group of workers hired by CSX was putting in new railroad ties when a construction train hit one of them. An emergency call from the scene, picked up on radio traffic from Broadcastify, had a caller shouting, “Emergency! Emergency! Someone just got hit.”
Las Cruces Police officer stabbed, killed in line of duty
EL PASO, Texas – A Las Cruces Police officer was killed Sunday afternoon, Feb. 11, in the line of duty after responding to a trespassing call, the Las Cruces Police Department (LCPD) announced in a press release. LCPD says that Patrol Officer Jonah Hernandez, from El Paso, who has been with the department for two years, was stabbed at least once after responding to a trespassing call shortly before 5 p.m. Sunday on the 300 block of South Valley Drive. Hernandez was transported to MountainView Regional Medical Center where he died from his injuries, according to LCPD. LCPD says the investigation is in its early stages, but police believe Hernandez was stabbed by the suspect who allegedly trespassed onto the property.
