2 workers killed in crane collapse on Everett, Massachusetts waterfront
Everett, MA — A large crane collapsed on the waterfront in Everett, Massachusetts on Friday afternoon, killing two workers. Skyeye was over the scene on Rover Street, not far from the Encore Boston Harbor casino, where the crane fell on a dock, and barge. Larriston Lake, 44, of Roslindale, and Paul Ledwell, 37, of Middleborough, were working at the former Exxon Mobile Corporation terminal on the Mystic River at the time of the collapse. Everett Mayor Carlo DeMaria said a cable snapped and the crane fell on workers cutting pipes off the dock. “Two young guys, you know 30s, 40s, working to provide probably for their families, lose their life at work,” DeMaria said. “I’m sure when they left home this morning that wasn’t even a thought.” One person died at the scene. Another was taken to the hospital with injuries and later pronounced dead. The body of one of the workers was removed from the crane hours after the collapse.
N.J. window washer fell to his death while on the job in New York City
New York, NY — A Hudson County man fell to his death on Sunday while working as a window washer in New York City, authorities aid. Ander Bustamante, 26, of Kearny, was found around 9 a.m. on the sidewalk in front of Columbia University’s School of Nursing at 560 West 168th Street, according to the NYPD. He was suffering from injuries that indicated he fell from a high point, police said. First responders rushed Bustamante to Columbia Presbyterian Medical Center, where he was pronounced dead, authorities said. It was not immediately clear what caused him to fall while he was working.

According to Moundville Police, the worker was injured around 11 a.m. on Saturday, Oct. 18 while completing work at Westervelt Lumber. A representative with The Westervelt Company says the worker was taken by ambulance to a local hospital where they died from their injuries. Police say they began investigating the accident on Sunday, Oct. 19 and are still working to find out what exactly happened. They are waiting to hear from the medical examiner in Montgomery but do not suspect foul play.
56-Year-Old Man Dead In Gas Well Explosion Near Jeffrey City
Jeffrey City, MT — A 56-year-old man died in a gas well incident near Jeffrey City on Wednesday that one fire chief described as an “explosion,” authorities say. The incident, described as an “industrial accident” happened around 3:55 p.m. near Jeffrey City along Crooks Gap Road in the southeastern part of the county, according to a Thursday statement by the Fremont County Sheriff’s Office. Authorities do not yet know the cause of the blast, according to the sheriff’s office. Jeffrey City Volunteer Fire Department Chief Vern Redland said Lost Soldier Oil and Gas, a Wyoming-based energy developer, was operating the well. The Fremont County Coroner’s Office declined to release the identity of the deceased, citing the ongoing investigation. Redland told Cowboy State Daily his responders were the first to arrive on the scene. He said the victim was likely an employee who attempted to turn off a valve after noticing a rupture in the gas flow line. While police used the term “blow-out” to describe the incident, Redland said the well is still operational. He confirmed the term “explosion” is accurate for this incident. SAN ANTONIO, TX — Officials in Guadalupe County are reporting the death of a contracted line worker. According to the Guadalupe County Office of Emergency Management and Fire Marshal, crews were called to Wetz Street in Marion Thursday afternoon to the report of an injured line worker. Authorities say the worker was 175 feet off the ground when he was electrocuted. Rescue workers were able to recover the man and CPS Energy crews arrived to secure the electrical hazards at the scene.
Funeral home worker crushed to death by burial vault in freak accident
Dallas, TX — A funeral home worker was crushed to death when a burial vault toppled on him in a freak accident at a Dallas mortuary Monday, according to local officials. Texas firefighters found the man around 2 p.m. pinned from the waist down by the vault, which had been stacked outside the Restland Funeral Home, Cemetery and Crematory, according to Dallas Fire and Rescue. Such vaults are often made of concrete to protect caskets from the elements after burial. The worker suffered serious injuries to the lower part of his body. Responders needed hydraulic spreaders and air bags — typically used in car-wreck extractions — to free the worker, who was rushed to the hospital with serious lower body injuries, NBC Dallas Fort Worth reported. He then died later that day.

Construction Worker Dies After Marquette County Traffic Crash
Sands Township, MI — A road worker is dead after a car hit him and two construction company trucks Monday in Sands Township. The Marquette County sheriff says it happened on M-553 near Silver Creek Road about 1:30 Monday afternoon. The driver was taken to U-P Health System Marquette for treatment. Jack Rantanen, 25, of Marquette was killed Monday after a crash. M-553 was closed for several hours. At approximately 1:36 p.m., Marquette County Sheriff’s Deputies were dispatched to the scene of a traffic crash in a construction zone in Sands Township. A car struck an employee working in the construction area, leading to significant injuries. Upon arrival, deputies assisted with lifesaving efforts alongside bystanders and coworkers. Preliminary investigation revealed that a southbound passenger vehicle left its lane, sideswiping a Hooper construction vehicle parked on the shoulder. The car continued to strike a worker who was securing equipment to a trailer being pulled by the construction vehicle. The vehicle then collided with a second Hooper truck that was parked off the roadway.

Liberty firefighter dies from ‘line of duty’ cancer diagnosis
KANSAS CITY, Mo. — In honor of Captain Raymond McElwee, a long-time Liberty firefighter who died from cancer last week, both the Liberty Fire Department and his family are inviting the public to celebrate his life at his visitation and funeral on Wednesday. According to the City of Liberty, McElwee passed on Wednesday, Oct. 22, after a long and courageous battle with pancreatic cancer for the majority of the year. His death was deemed a “line of duty death” because the city said his diagnosis was likely a result of his 26-year career in the firefighting field. “The loss of a firefighter is never easy, but the second in just over a year to the same kind of cancer caused by the job that we all dedicate our lives to, is particularly devastating,” said Fire Chief Chris Young.
Blairsville man electrocuted in Oct. 24 Somerset County accident
Blairsville, PA — Somerset County Coroner Cullen Swank has released the name of the man who was fatally injured Oct. 24 in a workplace accident. Scott Page, 52, of Blairsville, was pronounced dead at the scene, Swank said. The cause of death was electrocution, he said. According to the Somerset County emergency dispatch center, Sipesville and Acosta fire departments, along with Somerset Emergency Medical Service and Pennsylvania State Police Somerset, were called at 2:40 p.m. to the 2600 block of Keyerstown Road in Lincoln Township for a male in a bucket truck that hit power lines.
La Mesa officer killed while rendering aid in deadly crash on I-8 in Grantville
La Mesa, CA — A La Mesa police officer was killed on eastbound Interstate 8 in the Granville area late Monday while responding to a car crash that left another driver dead. La Mesa Police Department officer Lauren Craven, 25, was outside of her vehicle rendering aid for a deadly crash near Fairmount Avenue before 10:30 p.m. Monday when she was struck and killed by an oncoming vehicle, the California Highway Patrol said. A driver in the initial crash, identified by family members as 19-year-old De’Veonte Morris, also died. It began when two cars crashed on the eastbound I-8, Williams said. One of those vehicles overturned in the lanes and was struck by another car. Morris, who was in the overturned vehicle, died. That’s when Craven, who was on duty, stopped and got out of the car to render aid. Another car approached and hit the officer, as well as the other crashed cars. A fifth vehicle then slammed into the crash scene, Williams said.

HARRIS COUNTY, Texas – A man is dead after authorities say a forklift overturned on him, killing him. It happened around 3:45 p.m. at Dixie Foundation Drilling in the 5400 block of Killough Street, just off Antoine Drive. According to the Harris County Sheriff’s Office, a woman called and said a worker had flipped in the forklift. The licensed forklift operator, in his 50s, appeared to have been moving something heavy when surveillance video showed the forklift start to collapse onto him. EMS arrive and found him outside the building. He was confirmed dead at 4:12 p.m.
H-E-B warehouse employee killed after suffering possible forklift-related injuries
SAN ANTONIO, TX – A 27-year-old woman died after sustaining injuries possibly caused by a forklift at an H-E-B warehouse, according to San Antonio police. Officers said they were dispatched to the warehouse early Friday morning. The woman was operating her forklift when she came to a stop, a company spokesperson said. Another employee noticed she was showing signs of distress and called EMS. She was taken to a hospital, police said, where she was later pronounced dead. On Monday morning, SAPD identified the woman as Teresa Dominguez. Dominguez died of blunt trauma of the left lower extremity, and her death was accidental, the Medical Examiner’s Office said Monday morning. San Antonio police said in a preliminary report that they were investigating the “Apparent Sudden Death” that happened Friday in the 500 block of Rittiman Road, when the 27-year-old Dominguez “sustained an injury from possibly a forklift at the HEB warehouse, and was found unconscious inside of the freezer.”

Officers arrived at a metal fabrication shop and found a 50-year-old Wichita man with injuries to his upper body. He was operating a hydraulic press, and a piece of metal separated and injured him, according to the Wichita Police Department. The man was taken to a local hospital, where he died soon after. Washington County-Johnson City EMS member dies in the line of duty
Johnson City, TN — A high-angle rescue class turned horrific for students when the instructor suddenly plummeted to his death near a mountain waterfall, according to investigators in the mountains of northeast Tennessee. Advanced EMT instructor Fred Kemp was surrounded by fellow EMTs at the time, but their efforts could not overcome his traumatic injuries, Washington County/Johnson City EMS said in an Oct. 23 news release. New York, NY — A construction worker is dead after he fell more than 100 feet from a building into a construction hole at Hudson Yards on Thursday morning, according to the FDNY. The worker for the Hudson Yards Concrete Casing project was found at the bottom of the 60-foot hole near 12th Avenue and 30th Street around 8:53 a.m., fire officials said. He appeared to have fallen from at least 50 feet up the building, according to EMS Chief Nicholas Aiello. Contractor killed attempting to change locks on what was supposed to be a vacant home
FREDERICKSBURG, Va. (WUSA) – A Virginia family says they are trying to come to terms with the death of their oldest son, who was shot while trying to secure a home. “You killed my son for nothing. You killed him for nothing,” Lea Dodge, Michael Dodge II’s mother, said. “He was a good human.” According to the family, the 35-year-old was working as a contractor for mortgage companies. “For foreclosures and abandoned properties, he would, if the property was broken, go secure it. If the property had to have some minor repairs, he would do the repairs,” Michael Dodge’s father, Michael William Dodge, said. On Tuesday, Michael Dodge was said to be sent by his company to a home on Mountain View Road in Fredericksburg to secure the home and change the locks. But when he got there, Michael Dodge was confronted by a man. “Michael called the company that he works for and said, ‘Hey, there’s somebody in the property. They’re here, they’re being aggressive, and they have a gun on me,” Michael Dodge’s father said. “From my understanding, he was attempting to back away, and there were gunshots.” Michael Dodge’s coworkers called 911, police responded, but they reported finding him lying in the driveway dead. After a brief standoff, the suspected shooter, 64-year-old Donald Thomas, was arrested at the home and charged with first-degree murder.

Parker police shoot, kill Colorado gas station employee who pointed gun at officers, police say
Parker, CO — Six Parker police officers were placed on administrative leave after a shooting that left a Colorado gas station employee dead. Officers responded to reports of gunshots heard in the area of Jordan Road and Mainstreet after multiple residents called 911 around 10:20 p.m. Monday. When officers arrived less than three minutes later, according to investigators, they found spent shell casings and live ammunition in the parking lot at the T Square gas station located at 17050 East Mainstreet. That’s when officers said they contacted the gas station employee, who officers said initially presented himself as a witness to the shots-fired call. Officers said that the employee became very agitated with officers and was acting erratically. They also said he lifted his sweatshirt multiple times, showing a gun that was tucked into his waistband. Officers said they made numerous attempts to plead with the 32-year-old suspect to de-escalate the situation and surrender. That’s when officers said he became more agitated and grabbed his gun, pointing it at officers. Officers said they fired their weapons, striking the suspect.

Laredo, TX — A death investigation is underway in Laredo after a man reportedly died while working in a palm tree. Laredo police said at 3:53 p.m. that the road was being closed in the 100 block of Windsor Road to accommodate the investigation and medical examiners. According to police, the preliminary report indicates a man was part of a gardening company. He was working with a device up in the tree and died, possibly from a medical episode. Police said the fire department had to go up into the tree to get the man down.
State investigates after 2nd migrant worker at Nash Co. farm dies since 2023.
Nash County, NC — A legal migrant farm worker was fatally injured while working at a major sweet potato farm in Nash County, marking the second such death at the same farm in just over two years. Daniel de la Cruz, a 36-year-old from Mexico, was at work at Barnes Farming and FarmPak on the afternoon of Oct. 3. He was riding on the side of a large farm truck — between the trailer’s main cabin and the truck bed — on a state road close to the farm. The vehicle, a 2005 Freightliner, made a sharp right turn at an intersection. De la Cruz was thrown from the truck and subsequently run over by it, according to a collision report from the NC State Highway Patrol obtained by Carolina Public Press. He was taken to Wilson Medical Center, and eventually transferred to ECU Health Greenville, where he died on Oct. 10 as a result of his injuries. Casa Azul de Wilson, a nonprofit serving Eastern North Carolina’s Latino community, calls de la Cruz’s death a “horrific and preventable tragedy.” “His pelvis was crushed, and witnesses reported that his intestines were spilling out of his body,” wrote Flor Herrera-Picasso, Casa Azul’s executive director. “He was airlifted to the nearest hospital …. Despite multiple surgeries, Daniel ultimately succumbed to his injuries and passed away.” The NC Department of Labor is currently investigating the circumstances of de la Cruz’s death. That investigation is likely to turn on whether farm worker supervisors instructed de la Cruz to ride on the side of the truck, or at least knew — or should have known — he was traveling in that manner. De la Cruz is the second migrant worker to die while working for the farm in the last three years.

Semitruck driver killed in rollover crash on northbound I-25 in Loveland on Oct. 21
Loveland, CO — A semitruck driver died in a rollover crash on northbound Interstate 25 in Loveland the afternoon of Oct. 21. The semitruck was traveling northbound on I-25 about 2 p.m. when it went off the right shoulder and through the guardrail near milepost 258, between the Eisenhower Boulevard/U.S. Highway 34 and Crossroads Boulevard exits in Loveland. The semitruck rolled onto the driver’s side and the driver was ejected from the vehicle, according to a news release from Colorado State Patrol. The driver was pronounced dead at the scene. The Larimer County Coroner’s Office will release the driver’s identity and cause of death at a later time.

D’Iverville, MS — The City of D’Iberville announced the death of Davey Quave, an electrician for the Public Works Department. Quave had sustained injuries from a car accident on October 13 while traveling on Highway 15 to work on a city pump station. He died on October 18.
Semi-Truck Driver Killed in Fiery Crash on I-75 Near MM 370
Marion County, FL – Florida Highway Patrol troopers are working to identify the driver of a semitractor-trailer who tragically died in a fiery crash on Interstate 75 on the evening of Saturday, October 19, 2025. The fatal accident occurred shortly after 8:00 p.m. near mile marker 370, approximately two miles north of the County Road 318/Orange Lake interchange in Marion County. Emergency responders arrived to find the cab of the semi-truck completely engulfed in flames. Authorities confirmed that the driver, who was the sole occupant of the vehicle, was pronounced deceased at the scene and had sustained severe burn injuries. Due to the extent of the damage, the victim has not yet been identified. According to Florida Highway Patrol’s preliminary investigation, the big rig was traveling south in the right lane when, for reasons still under investigation, it veered off onto the right paved shoulder. Williams was an employee at Boatright Trucking from Alma, Georgia. Shortly after 8 p.m. on Oct. 19, FHP was notified of a crash on southbound I-75 by mile marker 370, which is 2 miles north of the County Road 318/Orange Lake interchange. Troopers said the heavy-duty vehicle was southbound in the right lane when, for an unknown reason, it veered off the interstate onto the right paved shoulder and struck the guard rail. The vehicle continued south onto the grassy shoulder and struck multiple trees. The truck jackknifed, with a part of the cab separated from the trailer.
7-Eleven worker killed in murder-suicide by man he didn’t know
VANCOUVER, Wash. (KPTV/Gray News) – Police are investigating a murder-suicide at an area 7-Eleven where a worker was shot and killed. Deputies and emergency crews responded to a 7-Eleven on Highway 99 after reports of a shooting on Oct. 17. When first responders arrived, they found two men inside the store with gunshot wounds. Both were taken to a hospital, where they were pronounced dead. According to the Clark County Medical Examiner’s Office, 26-year-old Ryan Taylor, of Vancouver, shot and killed 37-year-old Pardeep Kumar, also of Vancouver, before shooting himself..
