The man, identified as Pedro Gasca Alvarez, 41, of Carpentersville, was rushed by ambulance to Advocate Condell Medical Center in Libertyville, according to Lake County Chief Deputy Coroner Steve Newton. Gasca Alvarez had fallen headfirst approximately 12 feet off the roof of the single-family home, Newton said. The man had been above the front door area of the home working on the roof. Newton said Gasca Alvarez was pronounced dead in the intensive care unit of the hospital in the early afternoon on April 23. The Lake County Coroner’s Office was notified and preliminary results indicate Gasca Alvarez died as a result of head injuries. Rescuers from multiple agencies were called out to the scene at Mt. Hood Ski Bowl East around 9:30 a.m. The two injured individuals were employees at Mt. Hood Ski Bowl, according to the sheriff’s office. Authorities said the employees were in a maintenance basket when it fell from the upper-bowl chairlift into the rugged terrain below. Because of the remote and rugged location, crews used specialized equipment, including all-terrain vehicles and air medical resources, to reach and treat the victims. Police arrested 41-year-old Rene Pena-Diaz for murder in connection to the death. He is being held at the Jackson County Detention Center without bond. Officials said the case will be prosecuted in Jackson County. According to court records, an Immigrations and Customs Enforcement detainer has also been issued for Pena-Diaz. WICHITA, KS — Boeing is investigating an accident reported at its Wichita plant. 12 News learned that the accident happened more than a week ago and that the affected employee, identified by family as 53-year-old Daniel Lussier, died last weekend. As of Friday afternoon, it’s unclear whether the accident is tied to the employee’s death. The family said that on April 22, he stepped on a crossbeam and fell through some floorboards at the facility. “One of the kids was bringing him a track, he stepped on a crossbeam to get out of the way, his foot slipped and he fell,” described Daniel’s wife, Laina Lussier. The family reported that Daniel, who was an aircraft mechanic and was a Boeing employee for nine years, broke multiple ribs from the accident and was admitted to the hospital. He was released from the hospital on April 23, and the family said he died on April 26. Daniel’s family said they just want answers about the accident. They are waiting on the autopsy results before they know the official cause of death, but the family said they are asking questions about safety regulations at Boeing, and they have hired an investigator to look into the incident.
Postal Worker Dies After Being Pinned Under Overturned Delivery Truck

DeKalb County Police confirmed to PEOPLE that their officers responded to a reported crash at Larkspur Terrace and Iris Lane at around 7:11 p.m. on Monday. When they arrived, the officers found a two-vehicle collision between a postal vehicle and a passenger vehicle. The postal driver died at the scene, police said. According to a preliminary investigation, the passenger vehicle appeared to turn from Iris onto Larkspur when it struck the postal vehicle, which then overturned, police said. 34-year-old man killed in head-on crash while delivering DoorDash, family says
LOS BANOS, CA — This is the moment a routine workday in Los Banos turned deadly for 34-year-old Michael Qualls. “The initial 911 call came in around 8:10 of a two-car head-on crash,” Los Banos Sgt. Juan Soriano said. That Saturday morning, the collision occurred at the intersection of Santa Barbara Drive and Linwood Court, where Qualls’ family says he was delivering DoorDash orders. Exclusive video obtained by Action News shows a sedan flying down Santa Barbara Drive as Qualls is turning, and the two collide. One neighbor says the sedan was going around twice the speed limit for this residential area. “He was coming between at least 50 to 60 mph when he hit head-on with the other car,” neighbor Epifanio Gomez stated. Costco employee who was elderly mom’s only caregiver is shot dead, allegedly by senseless customer
STRONGVILLE, OH — A “selfless and caring” Costco employee, who was his elderly mother’s sole caregiver, was killed in a senseless shooting after he tried to stop a gun-carrying shopper from entering the Ohio big-box store on Saturday, new court records show. Randy Corrigan, 61, was working at the Strongville, Ohio, Costco last Saturday when witnesses say he told a man walking up to the entrance with a “drum magazine protruding from one of his pockets” that he could not enter the store with a gun, according to court documents obtained by FOX 8. The suspect — identified as 22-year-old Christian M. Bryant — took out a semi-automatic handgun with the drum magazine and fired a first round at Corrigan shortly before 5:45 p.m., according to the docs. “After a split-second pause, [Bryant] fired many more [shots], one after another, with no pause,” the court records said. Corrigan was struck multiple times, including in the chest, abdomen and right arm, according to the docs. Cops rushed to the Costco following the gunfire and found Corrigan breathing and conscious enough to answer questions — however, “[it] was apparent his alertness was beginning to decline,” the report said. Despite lifesaving efforts, the 61-year-old worker died from his injuries, according to the outlet.

SAN ANTONIO, TX — An adult staff member at Raba Elementary School who was struck by a personal vehicle Wednesday morning on school property has died, according to district officials. Northside Independent School District spokesman Barry Perez confirmed the incident happened at the campus in the 9700 block of Raba Drive on the northwest side of San Antonio. Perez said EMS crews responded to the scene, treated the employee and transported her to a local hospital. DALLAS, TX — Dallas Fire-Rescue crews were called to a medical emergency Monday afternoon at Love Field, where one person later died. Officials say units were dispatched around 2:51 p.m. to a report of an unconscious person on the roof of a six-story parking garage located at 8008 Herb Kelleher Way. The man was reportedly working on an HVAC unit at the time. DFR confirmed that he was unresponsive when crews arrived and required life-saving measures, including CPR. The rescue was complicated by the man’s location, the department said in a news release. He was found on a platform about 15 feet above the roof level where the HVAC units were installed. SOMERSET COUNTY, MD — Maryland State Police are investigating a fatal crash in Somerset County that claimed the life of a highway worker from Delmar. PALM DESERT, CA — The coroner’s office released the identity of a Yucca Valley man killed in a crash between a cement truck and a semi truck earlier this month on Interstate 10 in Palm Desert. George Mauricio, 62, of Yucca Valley was driving a cement truck westbound when a semi-truck on the opposite lane suddenly crashed through the center median. The semi then broadsided a cement truck, and after, while the semi was perpendicular on the freeway, it was struck by a Jeep. The semi truck also caused four vehicles in the eastbound lanes to be involved in crashes. Authorities said Mauricio was pronounced dead at the scene. The driver of the semi-truck, a 38-year-old man from Menifee, was taken to Desert Regional Medical Center in Palm Springs with major injuries. A total of six patients were taken to the hospital with a range of injuries, ranging from major to precautionary, CHP confirmed.
Septic truck driver killed in train crash on CR 621 in Dayton
DAYTON, TX — A 33-year-old Rosharon man, Kendall McNeal, was killed Wednesday afternoon in a collision involving a septic truck and a freight train in the Dayton area. The crash occurred at approximately 12:52 p.m. April 29 on County Road 621. According to the Sgt. Rob Willoughby, supervisor of the Liberty County office of the Texas Department of Public Safety, a 2017 septic truck for Texas Outhouse was traveling northbound on CR 621 when it approached a railroad crossing at the same time as a Canadian Pacific Kansas City freight train. Willoughby said the driver of the truck failed to stop for the oncoming train and was struck. The impact caused the tank to separate from the truck, and the cab rolled over into a nearby ditch. The driver of the truck was pronounced dead at the scene by Precinct 4 Justice of the Peace Larry Wilburn. An autopsy has been ordered. Authorities confirmed that next of kin has been notified. The name of the deceased is being withheld at this time to give time for the rest of his loved ones to be notified. The train engineer, a 35-year-old man from Houston, was not injured in the crash.

CATOOSA, OK — The Catoosa Police Department said one man was killed in a fatal workplace incident in Catoosa. Crittenden County Sheriff’s deputy Coyle was shot in the line of duty on April 2 in Sturgis and passed away from his injuries on April 30. His funeral is scheduled for 2 p.m. on Friday at Rocket Arena at Crittenden County High School in Marion, Kentucky. School leaders said the school closure is to allow students, staff and the community to pay their respects. Deputy Coyle served as the School Resource Officer for Crittenden County High School for two years. Before that, he served with the Chicago Police Department for 28 years. In a social media post, school leaders said Coyle “faithfully served and protected our schools, and he meant so much to our students, staff, and community.”
18-year-old in custody after high-speed chase, charged in deadly bank robbery
LEXINGTON, KY — The suspect in a deadly Kentucky bank robbery and shooting is now facing federal charges. Brailen Weaver, 18, was detained near Interstate 64 in Lexington Friday morning. Weaver is facing federal charges for armed bank robbery, use of a firearm in a crime of violence and causing death with a firearm in the course of the crime of violence, according to court filings. WKYT obtained an affidavit from a special agent with the FBI in the deadly bank shooting case. The agent wrote in the affidavit that the suspect, now identified as Weaver, entered the U.S. Bank on Chestnut Street at 1:57 p.m. on Thursday. The suspect immediately shot and killed 42-year-old Brian Switzer, of Jessamine County, according to the affidavit. The suspect then shot and killed a teller, identified as 35-year-old Breanna Edwards, of Madison County. The federal investigator said the suspect checked multiple drawers and then immediately left the bank.
NJ Building Inspector Suffered Medical Emergency Before He Died In On-Duty Crash, Family Says
LAKEWOOD, PA — According to a GoFundMe launched by Christine Miller, Ronald Miller Jr. suffered a medical emergency while driving on Thursday afternoon, April 30. He is survived by a young son. As previously reported by Daily Voice, Miller was heading south on South Cooksbridge Road in a 2013 Jeep Patriot registered to the Jackson Township Building Department when the vehicle went off the roadway and struck a tree near Westlake Boulevard just before 2 p.m. The vehicle struck multiple objects before coming to rest against a large maple tree, according to police. While officers were en route, they were told that CPR was already in progress at the scene. Miller was taken by Jackson First Aid Squad to Monmouth Medical Center Southern Campus, where he was pronounced dead after life-saving efforts, police said.
Beloved Livingston Parish crossing guard killed, hit by vehicle outside elementary school
DENHAM SPRINGS, LA — A crossing guard died after being hit by a vehicle at a Denham Springs elementary school on Thursday afternoon, according to authorities. Crossing guard Katy Wells was on duty after school at Gray’s Creek Elementary when she was hit by a truck driving northwest on Hwy. 16, a Livingston Parish Schools spokesperson said. She died as a result of the crash. Deputies arrested Darren Goudeau, 64, of Walker. He was booked into the Livingston Parish Detention Center on charges of vehicular homicide and DWI. The school system said Wells has worked for it and the sheriff’s office for 40 years, and was most recently a crossing guard for Gray’s Creek Elementary and Seventh Ward Elementary.
Oklahoma man charged after crash which killed We Energies surveyor
TROY, WI — A 65-year-old We Energies surveyor was struck by a car and killed while on the job on Highway 20 in Walworth County Thursday morning. The suspected driver, 50-year-old Raschad Johnson of Norman, Oklahoma, was arrested and charged with homicide by negligent operation of a vehicle. According to the criminal complaint, Johnson had a cellphone and a laptop connected to a power station running in his vehicle when he hit the surveyor on Highway 20 and Townline Road just outside of East Troy. The complaint also claims the driver directly behind Johnson noticed him weaving on Highway 20. The complaint said he was about to call and report Johnson for reckless driving moments before the surveyor, identified as a 65-year-old man from New Berlin, was killed. The surveyor and the We Energies crew were all wearing high-visibility clothing and had their yellow safety lights activated at the time of the crash, according to the complaint.
1 dead in I-77 semi-truck crash near I-20 interchange in Richland County
RICHLAND COUNTY, SC — The South Carolina Highway Patrol says one person is dead and another is in the hospital after a crash that happened along I-77 around 5:15 p.m. Master Trooper Hannah Davidson with the South Carolina Highway Patrol said the crash happened near mile marker 16, which is near the I-20 interchange in Richland County, around 5:15 a.m. Davidson said a 2020 Volvo semi-truck was heading south when it went off the road to the left and hit a guardrail. The truck also hit the support of an overpass at the location. The driver died in the crash, and the front-seat passenger was taken to an area hospital with unspecified injuries. The South Carolina Highway Patrol is still investigating the crash. The South Carolina Department of Transportation said bridge inspection crews reviewed the site after the crash and found only minor damage.
I-71 is back open after fatal semi-truck crash, fire under overpass in downtown Cincinnati
CINCINNATI, OH — All lanes of I-71 re-opened just after 5 a.m. Monday morning, hours after a fatal semi-truck crash and fire under an overpass in downtown Cincinnati near Heritage Bank Center. Assistant Fire Chief Matt Flagler said just before 7 p.m. on Sunday, they received multiple phone calls about a large semi-truck fire. Multiple calls gave different locations for the fire, leading to a confusing scene and delayed response, Flagler said. Crews eventually found the semi-truck on northbound I-71 near the Lytle Tunnel, he said. The crash did not involve any other vehicles, and Flagler said the semi-truck driver crashed into the wall of the tunnel. The driver was trapped in the cab of the semi-truck, leading to crews attempting to rescue and remove him. Once the man was removed from the cab, he was pronounced dead at the scene, Flagler said. The man has not been identified. Crews were able to put out the fire, which was contained to mostly the cab of the semi-truck and the front of the trailer. There were no hazardous materials in the trailer, but the fire caused diesel fuel to spill in the roadway, Flagler said.
