Train worker killed when guiding train out of railyard

Houston, TX — A train worker was killed Friday morning when he was struck by a train he was guiding out of a Harris County rail yard, authorities said.  A train was reversing out of the rail yard in the area of 1755 Terminal,  and an 18-wheeler was simultaneously crossing the tracks, according to a tweet from Sheriff Ed Gonzalez.  The train driver apparently struck the employee, possibly pinning him against the 18-wheeler.

49-year-old woman killed by front-end loader

SIOUX FALLS, S.D.  — The Occupational Safety and Health Administration (OSHA) is investigating a death involving a front-end loader Friday morning. Authorities say a 49-year-old woman is dead after she was struck by the machinery around 9:30 a.m. The incident happened on private property in the 4000 block of North National Avenue.

Suspect in custody after employee found dead at Jamis headquarters

NORTHVALE, N.J. — Prosecutors say a suspect is in custody and a homicide investigation is underway after a body was found at the headquarters office of G. Joannou Cycle Co. Inc., the owner and distributor of the Jamis Bicycles brand. Jamis announced that employee Jeanette Willem is the victim. The company said Willem, who police said was 43, was “a longtime and most valued member of our family.”

Vale Irrigation District employee dies when truck rolls

VALE, OR – An employee of the Vale Oregon Irrigation District was killed Wednesday when a dump truck he was driving along a canal overturned and rolled, according to Malheur County Sheriff Brian Wolfe. The victim was Jonathan Harrison, 26, of Vale. Wolfe said Harrison was a part of a crew from the irrigation district hauling material to shore up a canal bank

Maryland AAA Worker Killed In I-70 Crash After Stopping To Change Tire

MARRIOTTSVILLE, Md. (WJZ) — State troopers are investigating a crash that killed a AAA tow truck worker Thursday morning in Howard County.  The driver was 38-year-old Muhammad Shehzad. The crash happened around 11:30 a.m. on eastbound I-70 in Marriottsville. Maryland state troopers said that Shehzad had just finished changing a flat tire on the side of the road when a pickup truck hit him and his work van. Shehzad was wearing a safety vest and his emergency lights were flashing.

N.H. state trooper killed after tractor-trailer hits cruiser

PORTSMOUTH, N.H. — A New Hampshire state trooper was killed in the line of duty Thursday after a collision with a tractor-trailer. According to CBS Boston, Staff Sgt. Jesse Sherrill, 44, was working a construction detail when the truck slammed into his cruiser. Sherill was taken to a hospital where he was declared dead. The driver of the tractor-trailer was treated and released, according to the report.

York County man died as a result of blunt force injuries in workplace accident Monday night

East Manchester Township, PA — A York County man died Monday night as a result of blunt force injuries sustained during a workplace accident at Johnson Controls in East Manchester Township, according to the York County Coroner’s Office. Christopher Raul, 20, was pinned under a piece of equipment at around 7 p.m., according to a previous report. He was transported to WellSpan York Hospital, where despite life-saving measures, he died of his injuries. Raul’s manner of death was ruled accidental, according to the York County coroner.

65-year-old Green Bay man dies in Seymour workplace incident

SEYMOUR, WI – A 65-year-old man from Green Bay died Monday afternoon in what Seymour police are calling a workplace incident. Police and emergency medical services were dispatched just before 2 p.m. to the loading dock of pallet supplier Performance Corp., located in the 100 block of North Mainline Drive, where the incident occurred, Seymour Police Chief Isaac Schultz said.  Police are not yet releasing information on the man’s identity or his cause of death.

Truck Driver Killed In Eastern Iowa I-80 Crash

Cedar County, IA — A truck driver is dead after an eastern crash this (Wednesday) morning. The Iowa State Patrol says 38-year-old Reynold Madara of Woodward, Oklahoma was getting back on I-80 after securing a load in Cedar County, when his semi was rear-ended by a second semi. The crash happened in the westbound lanes of I-80, at mile marker 277 at 7:10 a.m. Both semis went into the north ditch. The driver of the second truck, 52-year-old Jan Bisek of LaGrange, Illinois was killed.

Truck Driver Killed in Rear-End Crash on Interstate 30 in Royse City, TX

Royse City, TX — October 26, 2021, a truck driver was killed after rear-ending slowed traffic along Interstate 30 in Royse City.  Authorities say the incident happened Tuesday morning on I-30 near the Circle Drive exit. Preliminary investigation suggests traffic was backed up and slowed in the area when the victim’s semi-truck rear-ended the trailer of another 18-wheeler. The impact then pushed the rear-ended truck into a car ahead of it. The driver of the truck that hit the trailer was pronounced dead at the scene.

North Dakota Highway Patrol identifies sugarbeet hauler who died in crash near Reynolds

Reynolds, ND – Robert Lutovsky was driving a 2007 International Harvester straight truck loaded with sugarbeets when the vehicle left the roadway and entered the south ditch, where it struck an approach, partially tipped over and caught fire, the North Dakota Highway Patrol said.

OSHA opens investigation after man dies at Centre County construction site

Spring Township PA — The federal agency that oversees workplace safety launched an investigation after a worker died Monday at a construction site in Spring Township, a spokesperson wrote in an email Tuesday. Anthony Laterzo, 27, received a fatal electric shock after an excavator struck an overhead power line, an Occupational Safety and Health Administration spokesperson wrote. County coroner Scott Sayers wrote in a news release that Laterzo was standing in a ditch when the electricity traveled through the equipment and into the ground. He died of high voltage electrocution, Sayers wrote. His death was ruled accidental. 

Construction worker killed in accident was about to welcome baby boy

NORTH MIAMI, Fla. – A construction worker’s tragic death in an accident Monday came just days before his baby boy is supposed to be born. Gabriel “Gabe” Alvarez was working at a construction site near the entrance to Florida International University’s Biscayne Bay Campus in North Miami when a piece of heavy machinery fell on him and another man. Alvarez died at the scene while the other victim was taken to a nearby hospital.

Burrillville man’s family demands answers after workplace death involving cement truck

BURRILLVILLE, R.I.  — Family members of a Burrillville man who was fatally injured at work say they still have many unanswered questions about how it happened. Matthew Maynard, 33, was killed Thursday morning at the Greenville Ready Mix company in Smithfield, a police statement said last week. According to the police statement, Maynard was performing maintenance on a cement truck when he suddenly got caught in the truck’s hopper. “I’ve been trying to get answers for how, why. But nobody is there to give me the answers at all,” his father, James Maynard, told NBC 10 News.

Butler County patrol deputy killed in crash

BUTLER COUNTY, Kan.  – The Butler County Sheriff’s Office said a patrol deputy, and part-time police officer for the City of Burns, died Monday afternoon in a two-vehicle crash. The crash happened just before 5 p.m. in the 5200 Block of NW Hwy 77, El Dorado, while 44-year-old Stephen Evans was on duty for the City of Burns. The preliminary investigation indicates Evans was driving the Burns Police Department vehicle southbound and attempted a U-turn on Highway 77 and was struck in the driver’s side door by a pick-up truck that was also traveling southbound on Highway 77.  

Fire claims life of Yuma County firefighter as he headed to fellow firefighter’s funeral

YUMA COUNTY, Colo. (KDVR) — A Yuma County firefighter died Tuesday afternoon when high winds changed the direction of a running fire, claiming his life. Larry Wyant, a firefighter with the Joes Fire Department, was headed to the funeral of another area firefighter when a combine sparked a fire in a cornfield, the department told FOX31. “Every volunteer firefighter stopped what they were doing to come together and help and get containment of this fire,” the Joes Fire Department posted on Facebook.

Dump Truck Driver Killed On Route 51 After Colliding With Train

JEFFERSON HILLS, Pa. (KDKA) – A dump truck crashed into a moving train, killing the driver, Michael Peters, 64, of Rock Cave and shutting down Route 51 for hours. According to police, just before 11 p.m. Monday, a dump truck collided with a train carrying petroleum gas.

Driver killed after semi lands on Tin Top Road

WEATHERFORD,TX — A Fort Worth man was killed after the truck he was driving struck a guardrail and flipped off of the interstate and onto Tin Top Road Thursday, Oct. 21. He was identified by the Tarrant County Medical Examiner’s Office as Craig Haney, 63, of Fort Worth.

Illinois police officer is shot and killed near St. Louis

PONTOON BEACH, Ill. (AP) — A police officer was shot to death Tuesday by a man at a gas station in an Illinois suburb of St. Louis, authorities said. Pontoon Beach Officer Tyler Timmins, 36, died after being taken to a hospital, Illinois State Police Trooper Jayme Bufford said. The officer was shot at around 8 a.m. at a Speedway gas station along Route 111 and was rushed to a hospital in nearby Granite City

Cape May rookie lifeguard, a Chester County teen, dies from injuries after wave flips boat

Cape May, NJ — A 16-year-old Cape May rookie lifeguard who was critically injured while performing a patrol in the ocean Thursday has died, the Jersey Shore town announced Saturday morning. Norman V. Inferrera III, 16, of Phoenixville, had been rowing a lifeguard boat in the ocean off Reading Avenue Beach when the craft was broadsided by a wave and flipped over. Inferrera was knocked unconscious, according to the statement from City Manager Michael Voll.

Powell woman dies while on firefighting assignment

Powell, CA — A firefighter from Powell died Oct. 11 in California while on assignment to fight a fire in the Inyo National Forest, according to a National Forest news release. Layla Bradley, 29, died while on an assignment to the forest.

Friends and family honor fallen Harris Co. Pct. 4 deputy at funeral today

HOUSTON, Texas (KTRK) — A Harris County Precinct 4 Deputy Constable, Deputy Kareem Atkins, who was shot and killed while working an extra security job has been laid to rest.  He had just returned from paternity leave when he was shot and killed.  Those who know the deputies say they were best friends. The three men worked together at the Precinct 4 Deputy Constable’s Office, and they all worked an extra security job together at a bar on the North Freeway on the morning of Oct. 16.  As they tried to make an arrest, investigators say a man ambushed them with an AR-15 before running away. Atkins was killed at the scene.

Viewing Held Sunday for Fallen Hollywood Officer Yandy Chirino

Miami Lakes, FL — An emotional viewing was held for 28-year-old Hollywood Police officer Yandy Chirino in Miami Lakes Sunday. Officer Chirino was gunned down while responding to reports of a man breaking into cars in the Emerald Hills neighborhood of Hollywood on Sunday, October 17. Chirino was only with the department for four and half years before his death.
