According to Prairie Du Chien Police, they were dispatched alongside Gundersen Ambulance Service around 6:43 a.m. to 801 North Marquette Road for a report of an injury. Officers and medics were brought to a victim who had suffered severe injuries from the incident. The victim died of their injuries at the scene. The victim has been identified as 57-year-old Trisha Jones from Harpers Ferry. 3M and employees cooperated with the initial investigation, and the incident will be investigated further by Occupational Safety and Health Administration (OSHA).

Initially, the search was hampered by concern about the plant’s structural integrity following the blast that sent flames leaping high above the building and scattering industrial debris nearby. Bradbury described the blast as a “seven-alarm hazardous materials” event. Four other workers were sent to the hospital as a precaution, but were uninjured and released. Though other workers were at the site, Wixon was alone at the time of the accident. According to officials, he was working near a large piece of machinery at the time. Wixon’s death remains under investigation by the Brookings County Sheriff’s Office and the Mine Safety and Health Administration.
ARDOT worker dies after being hit by vehicle on I-430
LITTLE ROCK, Ark. — A Little Rock man has died after he was hit by a vehicle driving in the northbound lane of I-430 on Monday afternoon. According to reports, 25-year-old Timothy Harris was a maintenance worker for ARDOT who had been on the job for about two months. He was called to clear debris in the roadway near exit 9 Cantrell after the Rodney Parham interchange. A spokesperson for ARDOT explained that Harris went out to clear the debris around 1:00 p.m. in a marked truck while wearing a safety vest. There was apparently a shovel in the roadway, and he went to move it out of the way. Witnesses said it looked like he misjudged his chances of getting back to the side of the road. He quickly threw the shovel out of the way, when one vehicle avoided hitting Harris and then turned in the opposite direction of the shovel. This is when a second driver hit Harris. He was then taken to the hospital where he died shortly before 10:00 p.m.

According to a Monroe County Sheriff’s Office spokesperson, an employee of Frerichs Saw Mill on New Highway 68 died after becoming trapped under a concrete barrier. Hyundai is describing this as a workplace accident that happened on Saturday morning. Bryan County’s deputy coroner says a 35-year-old man died after falling from a structure of what will be the paint shop at the electric vehicle plant. The paint shop was one of the first buildings to go vertical at the site following the plant’s groundbreaking back in October. Hyundai says the man fell from a three-story structure. But just hours before his retirement was set to begin, he died in a car crash. But while Ken Anderson was out delivering his last bit of mail before retiring, he hit slush on the road, rolled his jeep into the ditch and was ejected. He was alive long enough to call 911, but all dispatch heard was him saying, “Help me.” His wife says if not for the crash, he would have been home as an official retiree just hours later.
Cameron Police Officer shot and killed while responding to domestic dispute
Cameron, TX — A Cameron Police Officer was killed while responding to a domestic dispute late Wednesday night, according to the Texas Department of Public Safety. Sgt. Bryan Washko said Cameron and Rockdale police were called to the 700 block of N. Travis around 11 p.m. Wednesday. Washko said a woman called to report a shooting. By the time police arrived, the woman had managed to run to a nearby store, Washko said. She said she was grazed by a bullet was not seriously injured. Police later served a warrant at a residence in the 500 block of East 7th St, where a suspect reportedly opened fire on police. Police got into a shootout with the suspect, in which a long gun was used. One of the Cameron police officers was shot. He was taken to Baylor Scott and White Hospital, where he died, according to Washko.

RICHMOND, Va. (WRIC) — Police are investigating after an argument between two men resulted in a deadly shooting at VCU Medical Center. Around midnight on Wednesday, May 10, officers with VCU Police and the Richmond Police Department were called to the VCU Medical Center North Hospital for a report that shots had been fired inside. According to 8News sources, a fight between two hospital employees resulted in one pulling out a gun and shooting the other. Police confirmed one gun was located and seized. Police said that one man was shot and pronounced dead while the other man was taken into custody.
McDonald’s worker lures manager to door, then shoots and kills her
Moultrie, GA — A Georgia man is accused of shooting and killing three people, including his manager, state investigators say. Surveillance video shows Kentavious White, 26, called his manager to the door of a McDonald’s in Moultrie before shooting and killing her, according to the Georgia Bureau of Investigation. White then turned the gun on himself, agents said. Authorities responded to the restaurant May 4 and found their bodies inside, investigators said. The woman who died was identified as Amia Smith.

TULARE, Calif. – Tulare police have arrested a suspect accused of a homicide at a spa. Back on April 9, Tulare Fire crews were called to a fire at 7 Day Spa on K Street off of Tulare -Avenue. During their search of the building, they found an employee in one of the rooms who was unresponsive. She was identified as Xiuqun Yuan of Los Angeles.Police say she was taken to the hospital, where she died. Detectives and arson investigators found evidence of foul play in the building. Denali, Alaska — A Denali National Park and Preserve employee was killed in an avalanche he triggered while skiing in Alaska’s wilderness areas, which are home to the country’s highest peak, officials said Friday. According to the National Park Service, Eric Walter was found dead Thursday after emergency responders rushed to the general area of his last location. A software engineer at Google fell to his death at the tech company’s Manhattan offices, according to a report. A 31-year-old man plunged to his death on Thursday evening around 11.30pm, according to the New York Post, citing police sources. The man reportedly worked at the search engine’s office located at 111 8th Avenue in Chelsea. He was found on the street unconscious and taken to Bellevue Hospital, where he was pronounced dead. Handprints and footprints were reportedly found at an open-air terrace on the Google building’s 14th floor. The man left no note before the fatal fall. Thursday’s incident is the second reported death of a New York City Google employee in just over two months. Chatsworth, TN — A man was killed Thursday at a Chatsworth business. Police say the man was removed from a machine at Sprinturf by fellow employees Thursday afternoon. 2 people sought in connection to deadly shooting of Toledo store employee
TOLEDO, Wash. — The Lewis County Sheriff’s Office is asking for the public’s help in finding two gunmen who are believed to be involved in the killing of a Toledo store employee. Someone called the sheriff’s office just before 9 p.m. Wednesday to report that they saw two masked people run out of a store in the 100 block of Mulford Road in Toledo and get into a white extended cab Dodge pick-up truck. The person told law enforcement that the individual’s actions “appeared suspicious.” When deputies arrived, they found the worker dead inside the business.
North Carolina construction worker, a father of 4, killed in crash on I-40
RALEIGH, N.C. — A construction worker was killed Wednesday when an SUV plowed into an active work zone on Interstate 40, crashing into equipment. Around 3 a.m., the SUV hit and killed a worker before crashing into construction equipment, the State Highway Patrol said. The driver, Jeyson Alexander Murcia-Guillen, 20, of Smithfield, faces multiple charges, including felony death by vehicle. Claude Tyler Bryant, or C.J., 33, of Rocky Mount, was killed. Indiana State Police describe the events leading to the crash: “After the preliminary investigation, Troopers believe a White 2013 Volvo semi-truck pulling a car-hauling trailer was merging into the eastbound lane of I-64 from the Corydon exit. At the same time, a Red 2014 Volvo semi-truck, pulling an empty box trailer, was eastbound on I-64 approaching the same exit. As the white semi merged into the slow lane of the interstate, the red semi collided with the rear of the car hauler trailer. The impact of the crash then pushed both vehicles off into the emergency lane on the south side of I-64, where both vehicles came to rest.” The driver hauling the cars wasn’t seriously hurt. The red Volvo driver, later identified as Clarence E. Mitchell, 64, required “extensive” extrication by the Harrison Township Fire Department. He was airlifted to a hospital and later passed away.”

SUFFOLK, Va. (WAVY) – Suffolk police are investigating after a 7-Eleven clerk was shot and killed on the job Friday morning. According to police, the shooting happened around 12:30 a.m. Friday at the 7-Eleven located in the 800 block of Carolina Road, near Tyson Court and Horton Drive. The business is in a relatively isolated area just north of Suffolk Executive Airport. Two unknown subjects entered the 7-Eleven with handguns and started shooting at Ketron Exavier Smith, the store clerk. Smith, 20-year-old man, sustained multiple gunshot wounds. He was pronounced deceased on the scene.
Officials identify driver killed in crash, chemical spill on I-10 in Tucson
TUCSON, Ariz. — Officials have identified the driver who died in a crash that resulted in a nitric acid spill on Interstate 10 earlier this week in Tucson. Rick Immel, 54, of Nevada may have experienced a medical issue before the crash on Tuesday, DPS officials said during a news conference Thursday afternoon, adding that intoxication was not a factor. The rollover caused the trailer shell to crack open, sending plumes of the volatile chemical into the air. Following the accident, I-10 was closed in both directions and area residents were told to shelter in place. After more than a day, I-10 reopened and the shelter in place order was lifted Wednesday evening. Immel was hauling liquid nitric acid for Landstar System, Inc. and was headed to Theodore, Ala.
T-Mobile employee killed in armed robbery, shooting at Baltimore store identified
BALTIMORE, MD — A man died after being shot Sunday in an armed robbery at his workplace, a T-Mobile store in Canton, Baltimore Police said. Officers responded Sunday afternoon to the T-Mobile Store in the 2500 block of Boston Street, where they found the 23-year-old employee inside with a gunshot wound. Fabian Alberto Sanchez Gonzalez was taken to a hospital for treatment and died Monday, according to police. Police said two unidentified males entered the store, announced a robbery, and one of the suspects shot the employee. The suspects then demanded the belonging of several customers before leaving the store on foot.

Wes Wheatley with the Blackfoot Police Department told EastIdahoNews.com officers responded to S & G Electric Motor Repair in Blackfoot at 8:35 a.m. for a man injured on some machinery. “He was still alert and talking when officers were there,” Wheatley said. Bonneville County Coroner Rick Taylor said the man was taken to Eastern Idaho Regional Medical Center in Idaho Falls and died later in the evening due to his injuries. Taylor identified him as Robert Davis of Pocatello. Next of kin has been notified. Taylor explained Davis was caught in some type of industrial lathe, and that was the cause of his death. The death was deemed to be accidental.
Section of I-10 could take 2 days to reopen after deadly, fiery semi crash near Willcox
WILLCOX, AZ (3TV/CBS 5) — ADOT says it could take 48 hours to reopen westbound lanes of Interstate 10 after a fiery crash killed a truck driver and caused a significant closure from Willcox to the New Mexico state line. The crash happened just after midnight Wednesday on I-10 at milepost 331 near Willcox. According to the Arizona Department of Public Safety, a semi ran off the right side of the roadway, struck a guardrail, and then collided with a pillar at the State Route 191 overpass. One pillar buckled and the truck caught fire. The driver was killed. Their name has not yet been released. DPS officials later confirmed that no hazardous materials were being transported.

New Independence, MN — The wife of a man who died over the weekend in a rollover tanker accident in New Independence, Minnesota told FOX 21 they moved to the area two months ago for a fresh start and change of scenery. Xavier Henson, 31, died Saturday when the tanker truck he was driving on Highway 33 was unable to negotiate the curve heading onto Highway 53. The tanker rolled and exploded into flames.
Person Killed By Fallen Tree In Redding Prompts OSHA Investigation
REDDING, CT — A person was killed by a fallen tree on Marchant Road, according to Redding police. The accident occurred Friday, and the victim was first described as “seriously hurt.” Police Chief Mark O’Donnell confirmed the death on Saturday, but the exact time and details have not been released. The investigation is now under the auspices of the Occupational Safety and Health Administration, according to police.
Wisconsin sheriff’s deputy fatally shot in St. Croix County during DWI stop
GLENWWOD CITY, Wis. (FOX 90 – A St. Croix County deputy was fatally shot while responding to a call about a possible drunk driver near Glenwood City, Wisconsin Saturday night. The Wisconsin Department of Justice (DOJ) says the St. Croix County Sheriff’s Office received a call around 6:15 p.m. about a drunk driver in a ditch near Glenwood City, according to a DOJ press release. Deputy Kaitie Leising was shot and killed as she responded to a drunk driving call after a vehicle went into the ditch. The Wisconsin Department of Justice says the officer conducted field sobriety tests on the shooter, identified as 34-year-old Jeremiah Johnson, before returning to his vehicle. Authorities say Johnson was “evasive” during the interaction. After speaking for about eight minutes, Johnson “turned toward Deputy Leising, drew a handgun, and shot her.” The DOJ says Deputy Leising was able to return fire but did not strike Johnson.
